Objective:Comparative Analysis of efficacy of endovascular intervention andartificial vascular bypass surgery for the treatment of arteriosclerosis obliterans offemoral popliteal artery(TASC C lesion).Methods: Retrospective analysis92cases of arteriosclerosis obliterans of femoralpopliteal artery(TASC C lesion) patients in our department form2003to2013.In thesecases endovascular intervention group(45cases),artificial vascular bypass surgerygroup(47cases).Patients in the endovascular intervention group were treated bypercutaneous transluminal angioplasty and stent placement,.Patients in the artificialvascular bypass surgery group were treated by femoral popliteal bypass.After theseoperations,all patients were given anticoagulant、antiplatelet and other treatment byunified specification. The preoperative, postoperative respectively on the two groupsunderwent segmental artery pressure measurement and Doppler ultrasound limbpatency rate, the results of chi square test and analyze the difference.Results:92patients(92phishom limbs) obtain success without any seriouscomplications.The intervention group and common complications in7patients,3cases of hemorrhage,4cases of stent occlusion,0cases of puncture pointinfection,bypass group general complications in6cases, hemorrhage in3cases,artificial vascular occlusion in2cases, incision infection in1cases.All patientswere followed-up after6months,12months,30months. In the45atients in theendovascular intervention group, the ABI before treatment are for a mean of0.35±0.18, the patency rate was11%,15days after operation,6months,12months,30months respectively0.78±0.05,0.54±0.12,0.52±0.12,0.50±0.06,the patency rate was92%、70%、62%、54%.In the bypass group47cases, the ABI are with anaverage of0.32±0.12,the patency rate was13%,surgery after15days,6months,12months,30months respectively0.80±0.10,0.79±0.06,0.75±0.13,0.70±0.11thepatency rate was95%、83%、75%、66%.The efficiency difference of the two groupshave statistically significant(P<0.05).Conclusion:Lower extremity atherosclerotic occlusive disease is a multi factors andmulti gene disease;The recent efficacy of the endovascular intervention(PTA+Stent)and artificial vascular bypass surgery for the treatment of arteriosclerosis obliterans offemoral popliteal artery(TASC C lesion) are both better,however,the long-termefficacy comparison of artificial vascular bypass and endovascular intervention,theformer is better,so as in the allows of the patients`s condition,the artificial vascularbypass surgery could be the first choice for arteriosclerosis obliterans of femoralpopliteal artery(TASC C lesion).
