Objective: The purpose of this study was to evaluate CT scan, SPECTimaging and SPECT imaging combined with CT scan in the diagnosis ofbenign and malignant lesions in pulmonary, and to evaluate the early imagingand delayed imaging in the diagnosis of benign and malignant lesions inpulmonary.Methods:74patients were analyzed according the inclusion andexclusion criteria, the early imaging was obtained60min after intravenousinjection of18F-FDG, and the delayed imaging was obtained120min afterintravenous injection of18F-FDG. The data obtained for each patient wereanalyzed the accuracy, sensitivity, positive predictive value(PPV) andnegative value(NPV),and the data were calculated and compared with2test,and the statistical analysis was performed using SPSS13.0software.Results: The sensitivity, specificity, accuracy, PPV, and NPV in thediagnosis of lesions in pulmonary of CT scan were72.55%,78.26%,74.32%,88.09%, and56.25%, respectively. The sensitivity, specificity, accuracy, PPV,and NPV in the diagnosis of lesions in pulmonary of SPECT imaging were88.24%,52.17%,77.03%,80.36%, and66.67%, respectively. The sensitivity,specificity, accuracy, PPV, and NPV in the diagnosis of lesions in pulmonaryof SPECT imaging combined with CT scan were92.16%,86.96%,90.54%,94.0%, and83.3%, respectively.Of the26cases of dual-time phase imaging, the sensitivity, specificity,accuracy, PPV, and NPV of SPECT early imaging were78.95%,57.14%,73.08%,83.33%and50.0%, respectively; and the sensitivity, specificity, accuracy, PPV, and NPV of delayed imaging were89.47%,42.86%,76.92%,80.95%,60.0%, respectively. In the delayed imaging,3nodules that with novisual uptake showed18F-FDG uptake in various degree, and the finaldiagnosis were two cases malignant nodules and one case of benign nodule.Of the22cases of pleural effusion, atelectasis, the sensitivity, specificity,accuracy, PPV, and NPV of SPECT imaging were93.75%,66.67%,86.36%,88.24%and80.0%, respectively. The sensitivity, specificity, accuracy, PPV,and NPV of enhanced CT were57.14%,75.0%,63.64%,80.0%,50.0%,respectively.The6SPN with diameter <1.5cm,4of the lesions were diagnosedpositive in SPECT imaging, and the positive rate was about66.67%.Conclusion:1. SPECT18F-FDG imaging can detect glucose metabolic activity inlesions of diameter>1.5cm,and SPECT imaging combined with CT canpromote the sensitivity, specificity, accuracy;2.The positive rate decreased significantly in lesion with diameter<1.5cm in SPECT imaging3. The positive rate of SPECT imaging in the obstructive pneumoniacaused by tumor was higher than that of enhanced CT in early stage;4. Dual-phase imaging can not promote the diagnostic value of SPECTimaging;5. Delayed imaging can detect lesions with no significant uptake in earlyimaging, it may improve the sensitivity;...
