Analysis Of Clinical Characteristics Of Anti-glomerular Basement Membrane Disease

Objective:To investigate the characteristics of anti-glomerular basement membrane disease,in order to deepen the knowledge of the group of diseases, clinical diagnosis andtreatment to help improve early diagnosis, early treatment, reduce mortality andimprove prognosis.Method:Clinical and pathological data of19patients detected anti GBM antibodyassociated diseases in our department in recent3years were retrospectivelyanalyzed.Result:1、 Clinical manifestations of19cases of patients with varying severity.15casesshowed acute nephritic syndrome with rapidly progressive renal failure,anemia, no patients with normal renal function,3cases with pulmonaryhemorrhage, one case died during hospitalization.2、13/19cases (68.4%) patients were simply anti-GBM antibodies,3/19cases(15.8%) anti-GBM antibodies with anti-neutrophil cytoplasmic antibody(ANCA)-positive,3/19cases (15.8%) anti-GBM antibodies with certainanti-nuclear antibody (ANA) positive, including one case accompanied by ananti-SSa (++), anti-histone antibodies (++), one case of anti-52ka antibody(++), anti-histone antibodies (++), one case of anti-histone antibodies (++).3、18patients received a combination therapy of corticosteroids,cyclophosphamide, plasmapheresis,4patients with serum creatinine levelsless than500umol/L in the initial treatment，14patients with serumcreatinine levels more than500umol/L in the initial treatment.4、13cases were treated with renal biopsy examination,11/13cases ofcrescentic glomerulonephritis, crescentic glomerulonephritis associated withone case of acute tubular injury, one case of mild mesangial proliferative glomerulonephritis.5、11cases of patients with lung infection on admission and treated withantibiotics; Finally, maintaining dialysis patients were mild anemia.Conclusion:1、Clinical manifestations of anti-GBM antibody-related diseases are of varyingseverity.2、Renal immunopathology：Part of the patient’s performance is typical of IgG, C3linearly along the glomerular capillary climb like deposition. Part of the renalbiopsy results showed granular deposition along the capillary loops. Also there aresome other types of special pathology.3、Plasmapheresis, Corticosteroids, cyclophosphamide treatment of the disease hasa good effect. early diagnosis and timely intensive immunosuppressive therapy isessential for improving the prognosis of patients.4、The patients with ESRD resulted from anti-GBM antibody-related diseases havea mild anemia.5、Several patients with certain ANA antibody positive, considering many patientsseek for Treatment in other hospitals,relating to some drugs.
