Objective:Endometrial cancer is the common malignant tumors of thereproductive system, moreover, its incidence is second only to cervicalcancer.In2010,43470new cases of endometrial cancer occured in American,almost7950cases died of endometrial carcinoma. Along with living standardsenhancement, the number of overweight women is growing.Meanwhile,theincidence of endometrial cancer is rising.Endometrial cancer is more commonin postmenopausal women (70%),perimenopausal women20%-25%, about5%less than40years old.The gold standard for the diagnosis of endometrialcancer is hysteroscopy or dilation.Before the treatment.If we can estimate thedepth of myometrial invasionã€ervical stromal involvement,and lymph nodemetastasis, we will make up treatment and evaluation of prognosis easily. Thefeatures of Magnetic Resonance Imaging (MRI) is high resolution ratio of softtissue and multi aspect imaging technique, it is commonly used to determinethe staging of endometrial carcinoma.However,The accuracy〠specificity andsensitivity of MRI examination is diffierent.According to FIGO2009stagingsystem of endometrial carcinoma, we compare preoperative MRI staging withpostoperative Operative-Pathology Stage,At the same time, analyize of theaccuracy of MRI staging, evaluate the application value of MRI staging ofendometrial carcinoma.Methods:In this study,94cases came from the Fourth Hospital of HebeiMedical University, gynecological admitted on January1,2010to January1,2013.94cases of endometrial carcinoma were confirmed by postoperativepathology carried out Preoperative examination by MRI.Images were blindlyreviewed by two master docters.If the two rusults were diffierent,they candiscuss each other.Lastly the MRI staging were compared with theOperative-Pathology staging.Finally,According to FIGO2009,we can get the accuracy of which MRI judging the depth of myometrial nvasion.we willreceive the accuracy of which MRI estimating ervical stromal involvement aswell as lymph node metastases.Results: In accordance with FIGO2009, consistency check of MRIstaging and Operative-Pathology Staging was on going.The results as follows:K=0.2947,z=3.7546,P<0.05.And the result was statistically significantdifference between groups.MRI staging and Operative-Pathology Stagingwere consistent.this study show:MRI diagnostic accuracy of endometrialcarcinoma was88.3%(83/94).For the depth of myometrial invasion,MRIreached the accuracy of96.8%(91/94)ã€specificity of98.4%(62/63)ã€sensitivity of93.5%(29/31).For Cervical invasion,MRI reached the accuracyof95.7%(90/94)ã€specificity of96%(48/50)ã€sensitivity of95.5%(42/44).Forlymph node metastases,MRI reached the accuracy of96.8%(91/94)ã€specificity of100%(87/87)ã€sensitivity of57.1%(4/7).In94cases, only1case which is intestinal mucosal involvement (IV)was not checked out byMRI.Conclusions:Accuracy, specificity, sensitivity of MRI judgingendometrial cancer staging were very strong.Most of all,MRI determined thedepth of myometrial invasion and Cervical invasion.MRI examination shouldbe included in the routine examination of paendometrial cancer before theoperation.MRI staging can effectively guide to develop a appropriatetreatment plan for patients with endometrial carcinoma, it is worthy ofusing. |
