Objective:Explore the action of the4-0absorbable interrupted suture lpancreaticintestinal side-mucosa anastomosis and the5-0mucosal vascular anastomosis suturecontinuous suture pancreatic intestinal side-mucosa anastomosis in the prevention ofpancreatic fistula in the pancreaticoduodenectomy,Choose a more suitablepancreatojejunostomy.Method:Aretrospective analysis of the clinical data of50patients which visit theSecond Hospital of Jilin University General Surgery Medical Center and the ThirdHospital of Jilin University General Surgery underwent pancreaticoduodenectomy.Surgery using the most commonly pancreatic intestinal side-mucosa anastomosis.50patients was divided into two groups based on the pancreatojejunostomy suture: groupA:the4-0absorbable interrupted suture lpancreatic intestinal side-mucosaanastomosis,38cases; group B:the5-0mucosal vascular anastomosis suturecontinuous suture pancreatic intestinal side-mucosa anastomosis,12cases; Wherein,30males and20females,aged32-66years with an average age was51years. Basedon clinical symptoms,biochemical markers and preoperative imaging diagnosisandconfirmed by postoperative pathology: pancreatic cancer,27cases; bile duct cancer,15cases;4cases of duodenal cancer,pancreatic cysts (serous cystadenoma)4cases.Application of SPSS19.0statistical software for statistical analysis of the data,Forcount data using one-way ANOVAvariance and rank sum test analysis,measurementdata analysis using the chi-square test. Comparison of preoperative cases in eachgroup(Age,sex,albumin,ALT,and bilirubin),Postoperative complicationrate(Pancreatic fistula,biliary fistula,Gastric emptying disorder,abdominal infectionincidence),the difference between the length of hospital stay.Results:1、The comparison of preoperative situation:group A,23males,15femaleswhich theAverage age was51.29years old;group B,7males,5females which theAverage age was51.25years old. P (Age)=0.989>0.05,P (albumin)=0.181>0.05,P (direct bilirubin)=0.771>0.05,P (prothrombin time)=0.226>0.05,P (preoperativeALT)=0.178>0.05.2、Postoperative recovery time comparison: P (recovery time)=0.030<0.05.3、Postoperative complication rate comparison: P (pancreatic leakage)=0.053>0.05,P (gastroparesis)=0.286>0.05.Conclusion: the4-0absorbable interrupted suture lpancreatic intestinal side-mucosaanastomosis applicable to any case of residual pancreas,the damage to pancreatictissue is little,the stump blood supply is not easy to destroy,and easy to operate,saving time,the complication rate of postoperative pancreatic leakage andgastroparesis is low,is an ideal choice in pancreatojejunostomy suture.
