The research backgroundTB (Tuberculosis,TB) is a Mycobacterium tuberculosis (MycobacteriumtuberculosisMTB) chronic infectious disease caused by infection by M.tuberculosis can infect multiple body systems, the most common is tuberculosis.According to statistics,the current population of the world has1/3of TBpatients[1], there are8million~10million patients eventually develop intoactive TB patients every year,about200to300people die of TB every year[2].In recent years,multi-drug resistant tuberculosis (MDR-TB) epidemic and thespread of human immunodeficiency virus (human immunodeficiency virus,HIV)infection and the emergence of global issues such as population movements,sothat the worsening TB epidemic,has become a serious social and public healthproblems,TB prevention and control is particularly important.Method and PurposeClinical study select from August2013to April2014175cases of patientswith suspected Japanese Friendship Hospital of Jilin University Respiratoryincome tuberculosis,including pneumonia,cancer and other diseases,and feverof unknown origin.Collect the patient’s pleural fluid,bronchoalveolar lavagefluid and pleural effusion refrigerated at4℃refrigerator box to be detected.Proper use of TB detection kit (Colloidal Gold),TST detection method,PCRmethod and compared with the final clinical diagnosis,by coincidence ratecomparison of three detection methods,the sensitivity,specificity,positivepredictive value,negative predictive value,positive likelihood ratio,negativelikelihood ratio,to further explore these three detection methods in thediagnosis of active tuberculosis.The results of46cases diagnosed with active TB,the colloidal gold,TST,PCR detectionsensitivity rates were73.9%,67.4%,45.7%,110cases of tuberculosis in non- active tuberculosis,three detection methods and specificity were80.9%,47.3%,86.4%,for the identification of active tuberculosis and non-compliance rate of active TB was78.8%,53.2%,74.4%,respectively.Thepositive predictive value of three detection and diagnosis of active tuberculosiswere61.8%,34.8%,58.3%,and negative predictive value were88.1%,77.6%,79.2%, positive likelihood ratio were3.87,1.28,3.36,negative likelihood ratio0.32,0.69,0.63.conclusion1.Colloidal gold has a strong diagnosis of active tuberculosis andtuberculous pleurisy capabilities.2.Colloidal gold assay was significantly better than TST.3.Colloidal gold method is superior in the diagnosis of active tuberculosisPCR detection sensitivity aspects,the overall diagnostic capabilities and PCRare similar,but the former is simple,fast,and less influential factors, with a widerrange of clinical applications.
