Objective: To compare the intramedullary needle thefixation offemoral shaft fracture of neck of femur and plate fixation offemoral shaftfracture, cannulated screw fixation of femoral neck fracture.Methods: We retrospectively reviewed25patents with ipsilateralfemoral shaft and neck fractures from December2010to May2013.According to treatment methods are divided into reconstructionintramedullary needle group of15cases. fixation of femoral shaftcombinedwith cannulated screw fixation of femoral neck in10cases. Wecompared two groups in operation time, bleeding in operation,postoperative fracture healing time and postoperative complications(delayed union, infection, implant loosening, femoral head necrosisor bone nonunion) were comparatively studied. Follow up of6months to2years. Average15months, two groups of patients were record ge-nder, age,fracture type, operation time and hemorrhage, postopera-tive complication and postoperative knee function score, and statisticalanalysis of two kinds of methods.Results: all the25cases were followed up, the time for6-24months,average15months, all cases obtained bone union standa-rd of two groups of femoral shaft fracture and femoral neck, at the end offollow-up no failure of internal fixation frac ture, loose ning, infect-ion, without the occurrence ofosteonecrosis of the femoral head, fracture delayed union and nonunion.Visible compared with intramedullary nail g-roup, plate group operation time is long, the amount of intraoper-ativebleeding, femoral diaphyseal long healing time. There were signifi-cant differences between them (P <0.05).Conclusion:combined diagnosis of ipsilateral femoral neckfracture infemoral neck fracture patients are usually neglected inthe femoral shaft.Therefore, attention should be paid to the diagno-sis and in femoral shaft fracture, still need to undertake physicalexamination and auxiliaryexamination of the hip, in order to avoid misseddiagnosis of femoral neck fracture. Treatment should be based on thegeneral condition of the patientand the type of fracture using asuitable internal fixation. Combined comparison, we considers thereconstruction is the treatment of femoral intramedullary nail, a betterway on1/3of ipsilateral femoral neck fracture; and plate fixation offemoral shaft fracture with cannulated screw fixation of femoral neckfracture, this approach can be widely applied all fracture type. Twofixation in patients with bleeding, surgery length, slight differences in thepostoperative recovery time; But in fracture healing, postoperative Harrisscore regard, no difference.
