Objective To evaluate the identification value of methylene blueand carbon nanoparticles as lymph tracer in the sentinel lymph nodeBiopsy(SLNB)for papillary thyroid carcinoma.Methods130patients with papillary thyroid carcinoma wereenrolled in the study,The patients were devided into two groupsrandomly.Two groups were given sentinel lymph node biopsy withmethylene blue and carbon nanoparticles respectively.The sentinel lymphnodes locations and amounts were recorded,followed by ipsilateralfunctional cervix lymph node dissection. Then the sentinel lymph nodeand dissected cervix lymph nodes were presented to pathologyexamination respectivelyResults In65cases of methylene blue group,52SLN cases wereidentified,27cases were histopathologically positive in lymph nodes. The detection rate, sensitivity,accuracy rate and false negative rate were80%,84.38%,90.38%,15.62%, respectively,8cases’s parathyroid weredyed,the dyeing rate was12.31%,2cases had post-operative lowcaciulm convulsions,the incidence was3.08%.while in carbonnanoparticles group,61SLN cases were identified,30cases werehistopathologically positive in lymph nodes. The detection rate,sensitivity,accuracy rate and false negative rate were93.84%、90.91%、95.08%、9.09%, respectively,1case’s parathyroidwere dyed,the dyeing rate was1.54%,no case had post-operative lowcaciulm convulsions,the incidence was0%。.The difference of thedetection rate and the parathyroid dyeing rate weresignificant(P<0.05),no significant difference on thesensitivity,accuracy rate, false negative rate and the incidence ofpost-operative low calcium convulsions.There was no significantlydifference on the distribution of the detected SLN in regions between twogroups.Both of the detection rate and positive rate of SLN are highest inregion Ⅳand Ⅵ in two groups respectively,no significant differencebetween two groups(P>0.05).Conclusions Intraoperative SLN identification with methylene blueor cabon nanoparticles is feasible,which could reflect lymphaticmetastasis accurately and be instructive to cervix lymph nodesdissection.Carbon nanoparticles method not only guarantees clearer surgery field but also gets higher detection rate,it is wothy of promotion.
