Objective: To compare extent of tumor resection, postoperative complications,KPS score between unilateral subfrontal approach and bilateral subfrontalapproach and evaluate their difference in curative effects. Methods: This paper retrospectively studies21large and12giant olfactorygroove meningioma patients to take unilateral subfrontal approach and bilateralsubfrontal approach as treatment. Preoperative MRI scans with tumors of4.0cm-6.0cm diameter were classified as large meningiomas and those morethan6.0cm diameter as huge meningioma. KPS score before and after surgerywere recorded, the Simpson assessment for extent of resection was used andstatistical analysis were carried out. Results: Using SPSS16.0software to process data showed unilateral subfrontalapproach group and bilateral subfrontal approach in the group had no statisticalpreoperative tumor size and age structure statistical difference. Extent of tumor resection, postoperative complications, KPS score before and after surgeryhad no statistical difference. Conclusion: For resection of large and giant olfactory groovemeningioma, unilateral subfrontal approach can achieve full cut levels similarto bilateral subfrontal approach. The incidence of postoperative complications, has no relation with the surgical approach and multiple factors mustbe considered before choosing operation approach to achieve good results.
