The Emergency Investigation Of Toe-to-thumb Reconstruction In Children

ObjectiveTo explore the difference in function of toe-to-thumb reconstructioncompared pediatric emergency surgery with elective surgery; Find the operationplan of toe-to-thumb reconstruction in pediatric emergency operation；And findsuperiority, safety, and clinical application value in emergency reconstruction.MethodsConcluded30children cases from January2010to January2014in ourhospital receive emergency toe-to-thumb reconstruction (emergency surgerygroup),compare with the32children cases (elective surgery group), review thedifference in the operation time, blood vessels, crisis rate, survival rate, handfunction and sensory function recovery.Results1. The emergency surgery group of30cases the average operation time was(122±12) minutes,1case of venous crisis occurred, the survival rate of100%,after6~24months follow-up,1case occurred bone absorption, reconstructionthumb appearance is satisfied, sensitive to return to activity, thumb, gripping,pinching is powerful. The elective surgery group32cases the average operationtime was (189±17) minutes,1case of arterial crisis occurs, the survival rate is 100%, no bone absorption. According to the association of university learn tothumb or finger renew function evaluation standard trial evaluation, theemergency surgery group fine rate is93.3%, higher than the elective surgerygroup62.5%, the experimental group sensation recovery is better than theelective surgery group.2. No group took place the foot complications; little impaction on thedevelopment of foot, the children could to walk, run and jump.Conclusion1. Pediatric thumb defect in emergency reconstruction can decrease theoperation time, reduce the risk of surgery, without second stage surgery.2. The application of digital technology auxiliary emergency operationmake the operation the more safer and reliable，Compared with elective surgery，the prevalence of vascular crisis and the survival rate have no obviousdifference.3. Emergency operation can save more important tissues，such as bloodvessels、nerves、tendons、bones and joints, the reconstruction fingers couldobtain satisfaction appearance, function, and sensory recovery is better than thatof elective surgery. It is worthy of clinical popularization and application.
