| Objective To study the changes in expression of serum calcium-bindingprotein S100A8(Calcium binding protein S100A8, S100A8) andlipop-olysaccharide binding protein (LPS-binding protein,LBP)before andafter mechanical ventilation, Understand their relationship withventilator-associated lung injury, Provide experimental basis for furtherunderstanding of the biological mechanisms of VALI.Method (1)20cases of lung cancer patients undergoing thoracotomylobectomy were devided into two-lung ventilation (two-lung ventilation, TLV)group and one-lung ventilation (One lung ventilation, OLV) grou-p atrandomized controlled case study method,10cases in each group.(2)Inclusioncriteria:patients with normal lung function or only mild ventilatory dysfunction;liver and renal function was normal;cardiac fun-ction I-Ⅱlevel.Case collectionprocess required to remove followingthe conditions::pulmonary ventilation process for any reason unable to maintainoxygen saturation above90%, such as:double-lumen endotrac-heal tube pooralignment restrictions and and postural restrictions;OLV time less than2h ormore than4h; bleeding too much, great hemodynamic fluctuations;use of hormones or vasoactive drugs intraoperative; patients with severe hypoxemia inanesthesia resuscitation, can not be successfully extubated.(3)Each patientusing the same method of anesthesia, anest-hesia machine and monitoringequipment, operated by the same anesthesiologist.(4) The central venous blood5ml extracted from each patient in the following four point: before anesthesia(T0), at the end of surgery (T1),24h after surgery (T2) and48h aftersurgery(T3),then placed in a blood coagulant tube, to be coagulated,centrifugedat3000rpm/min speed20min within4℃constant temperature, took theupper serum, stored at-80℃.(5)The S100A8and LBP in serum were testedby immunoblotting (Western Blot, WB).Results There was statistically significant difference about S100A8between the two groups at each time point (P <0.05); The two groups at eachtime point compared with T0,T1, T2, T3point difference was statisticallysignificant (P <0.05);There was no statistically significant difference about eachtime point of the two groups(P>0.05). There was an interaction betweengroups and between the time points,that the two groups differ in themagnitude of elevated S100A8, OLV group had a bigger rise over the TLVgroup.Conclusion S100A8and LBP may be involved in the process of VALI;OLV is still a safe way of ventilation. |
