| Objective:Heart failure is a variety of cardiac structural or functional disease cause ventricular filling and (or) ejection ability damage caused by a group of syndrome.Has to recognize heart failure is a disease of development, is accompanied by myocardial diastolic function of progressive decline of systematic, systemic diseases, internal mechanism including neuroendocrine, circulation of endocrine, inflammatory markers, metabolic hormones and other continuous activation and imbalance.NT-proBNP is secreted by the ventricular muscle cells without bioactive peptides containing76amino acid residues, long half-life (120min), in vitro stability is strong, effective testing time is long, is not a change and the impact of daily activities, more suitable for clinical application and is helpful to the diagnosis of heart failure.Plasma NT-proBNP in the diagnosis, risk stratification and prognosis of heart failure assessment and guide therapy research is a hot topic in recent years.This study mainly discusses the plasma NT-proBNP and the diagnosis of heart failure, cardiac function relation, and hospitalization rates within6months after discharge.Methods:1. The object of study:select January2013-2013,106patients with heart failure during hospitalization in October as the observation group, physical examination of50cases as control group.1.1HF group according to the NYHA heart function classification standard is divided into three subgroups:cardiac function level III class II group, and cardiac function, cardiac function level IV.1.2HF group according to left ventricular ejection fraction of different can be divided into three groups:group LVEF value30%or less, LVEF value30%45%group, LVEF value45%or more groups.1.3HF group according to different discharge standard is divided into two subgroups:Control group:after treatment for the standardization of positive, based on clinical symptoms and signs disappeared discharge standards group; observation group:the hospital in addition to the above standard, the plasma NT-proBNP concentration decreased30%or more groups.2. By using the statistical software SPSS13.0heart failure group and control group were compared between the plasma NT-proBNP concentration difference; The plasma NT-proBNP concentration and the relationship between the cardiac function and left ventricular ejection fraction; Heart failure, according to the standard of discharge between different subgroups within6months after hospital discharge of hospitalization rates again.3.Specimen collection:All began treatment for heart failure patients in the hospital before, when I left the hospital early in the morning on an empty stomach, the hypothesis, resting state under taking blood samples.Control group according to the same standard check-up on the day of blood samples.4.Measurements:All patients with plasma NT-proBNP concentration, left ventricular ejection fraction and heart function classification.Results:1.NT-proBNP concentration hf group was obviously higher than that of healthy controls, the difference is statistically significant (P<0.05).2.Concentration of plasma NT-proBNP follow function in patients with heart failure level (NYHA classification) higher and higher.3.Heart failure group plasma NT-proBNP concentration and negatively correlated with left ventricular ejection fraction.4.To discharge plasma NT-proBNP concentration is higher than the30%drop in for the standard group, hospitalization for heart failure again within six months rate significantly decreased.Conclusion:Plasma NT-proBNP concentration can reflect the change of the ventricular function, in the diagnosis of heart failure, heart function grading and prognosis are very important reference basis. |
