Comparison Of Cemented And Cementless Fixation In Total Knee Arthroplasty Of A Meta-analysis

Background It is not clear which fixation about cemented or cementless in total knee arthroplasty obtains the best clinical, functional and radiographic results.Objective To assess the effect difference of cement prosthesis and cementless prosthesis fixation in total knee arthroplasty based on only published reports that were randomized, controlled trials by Meta-Analysis.Methords The references at database at home and abroad were searched for the randomized controlled trials on cement prosthesis and cementless prosthesis fixcation in total knee arthroplasty. Screening according to the predetermined its inclution and exclusion criteria to literature, choose suitable for literature, quality evaluation, extract the implant survival rate, clinical score、function activity related to postoperative complications and radiology index and other related data. RevMan5.0software was used for Meta-analysis.Results In this review,586studies were identified and used, comparing cementless to cemented TKA. Totally1205cases form eight randomized controlled trials were involved. The cases were divided into the experimental group (cementless group) and the control group (cement group),568cases in the cementless group and637cases in the cement group. Meta analysis results suggest the bone cement TKA prostheses postoperative survival rate (p=0.12) related complications (p=0.88) advanced radiation bright line index (p=0.47) and no statistically significant difference were observed in the bone cement group, and clinical score (p=0.009) function activity (p=0.005) and bone cement group difference was statistically significant.Conclusion TKA surgery in the choice of bone cement prosthesis or bone cement prosthesis fixed the postoperative complications of prosthetic survival impact radiation bright line indicators have no obvious progress, rather than bone cement prosthesis in clinical scoring and functional activity of bone cement fixation group. So on the premise of good grasp indications in clinical application of cementless prosthesis fixation is a reliable choice.
