OBJECTIVE:To detect the serum and urinary iodine, serum and urinary total BPA (bisphenol A) level in the patients with papillary thyroid carcinoma or nodular goiter. To ensure the differences of BPA and iodine concentration among patients who had papillary thyroid carcinoma, nodular goiter and the controls. To investigate the relationship among iodine excess, environmental endocrine disruptor and the two diseases mentioned above that propose new foundations for preventing the two diseases.METHODS:All objects with informed consent of the study live in Shandong Province. The serum and urine samples were collected from the patients who had thyroidectomy from February2013to September2013in Qilu Hospital of Shangdong University. While the serum and urine samples of whose thyroid hormones and thyroid ultrasonography were normal were collected as normal controls. The coupled serum and urine samples are divided into three groups:papillary thyroid carcinoma (PTC) group, including53cases,14males and39females, age from20to81; the nodular goiter group, including60cases,14males and46females, age from21to70; the normal control group, including65cases,22males and43females, age from22to83. Detect and compare the iodine and bisphenol A levels in serum and urine of each group respectively. The iodine is detected by the WST-2006urine iodine of arsenic cerium catalytic spectrophotometry. The bisphenol A is detected by high performance liquid chromatography-tandem mass spectrometry. RESULTS:1. The urine iodine level:the urine iodine level is233.64±51.09ug/L、300.75±154.24ug/L、355.33±289.6ug/L in normal control group, the nodular goiter group and the PTC group. The urinary iodine concentrations of PTC group and nodular goiter group are higher than the control group. There are obviously significant differences between them. There is no statistically difference between the PTC group and the nodular goiter group. However, the urine iodine level in PTC group is higher than the nodular goiter group. According to WHO:iodine adequate is100-199ug/L, iodine more than adequate is200-300ug/L, iodine excess is>300ug/L。There is no iodine deficiency. The proportions of urinary iodine concentration in the PTC group and the nodular goiter group are different from the normal group. The proportion of iodine adequate in normal group is the highest, while the proportion in nodular goiter is higher than the PTC group. The proportion of iodine more than adequate in normal group is higher than both the PTC group and the nodular goiter group. The proportion of iodine excess in PTC group is the highest, and the nodular goiter group is higher than the normal group.2. The ratio of urinary iodine and creatine:the ratio is322.05±304.71ug/g,595.03±608.49ug/g,495.88±355.72ug/g in the normal control group, the nodular goiter group and the PTC group. There are obviously significant differences among them. The ratios in the PTC group and the nodular goiter are significantly higher than the normal group. However, there is no difference between the PTC group and the nodular goiter group.3. The urinary BPA level:The detection rate in PTC group, the nodular goiter group and the normal group is96.23%,86.67%,69.23%, while the urine bisphenol A level is6.78±6.45ng/ml,7.83±5.89ng/ml,4.69±3.06ng/ml. The urinary BPA levels and detection rates in the PTC group and the nodular goiter group are higher than the normal group. There are obviously significant differences between them. However, there is no statistically difference between the PTC group and the nodular goiter group. 4. The ratio of urinary BPA and creatine:the ratio is respectively7.55±6.10ug/g,13.74±12.11ug/g,6.49±5.80ug/g in PTC group, the nodular goiter group and the normal group. There is no statistically difference between the PTC group and the normal group. While there are statistically differences among the nodular goiter group and the PTC group, the normal group that the nodular goiter group is higher than the others.5. The iodine level in serum:the serum iodine level is respectively85.80±25.48ug/L,83.74±22.69ug/L,91.28±21.34ug/L in PTC group, the nodular goiter group and the normal group. There are no statistically differences among them.6. The BPA level in serum:the serum BPA level is respectively7.79±7.45ug/L,7.29±1.80ug/L,8.13±2.82ug/L in PTC group, the nodular goiter group and the normal group. There are no statistically differences among them.7. The correlation factor analysis:the iodine in serum is not correlate with urinary iodine, as well as the BPA。The urinary iodine concentration and the urinary BPA level are positively correlated, the same to the ratio adjusted by urinary creatine.CONCLUSION:1. The urinary iodine level in the PTC group and the nodular goiter group is significantly higher. The intake of excess iodine may take part in the pathogenic process of benign or malignant thyroid nodule. It indicates that we can reduce the intake of iodine to prevent papillary carcinoma and nodular goiter.2. The urinary BPA levels and detection rates in the PTC group and the nodular goiter group is significantly higher that prompt BPA, one of the environmental endocrine disruptors, having something to do with the papillary thyroid carcinoma and thyroid nodular goiter.3.The urinary iodine and BPA level are positively correlated that indicate both of them may contributing to the papillary thyroid carcinoma and nodular goiter coefficiently.
