Background:In the past many years, pancreatic tumor resection include:(1)resection of pancreatic body and tail joining spleen (2) Pancreaticoduodenectomy. But for low-grade malignant tumor of the pancreas and pancreatic benign in terms of.Not only taking into account whether the tumor can be completely removed all, but also we should be concerned about the quality of life of patients after a longer survival time. Such these two surgical approaches, normal pancreatic tissue and spleen are damaged. Due to advances in surgery now, The mortality rate has dropped to5%or less,but there are still up to30%complications. Now, people are not only concerned about survival and quality of life after more concerned. Retention function and minimally invasive surgical approach has gradually been carried out. Middle pancreatectomy is removal of pancreatic tumors to retain as much as possible on the basis of normal pancreatic tissue, attention to postoperative digestive function and blood glucose regulation.Objective:To discuss the value of the middle pancreatectomy in the treatment of pancreatic tumor of benign and low malignant pancreatic tumors located in the neck and the body.Methods:To collect the clinical data of15patients making middle pancreatectomy during February2010to February2014. And telephone follow-up.Results:Six men, nine women, average age40years old. Surgical approach:(1) Twelve cases middle pancreatectomy and Roux-en-Y anastomosis of pancreatic and jejunum.(2) One cases middle pancreatectomy and Roux-en-Y anastomosis of pancreatic and jejunum and nutritional jejunostomy.(3) One cases middle pancreatectomy and Roux-en-Y anastomosis of pancreatic and stomach.(4) One cases middle pancreatectomy with laparoscopy.Nobody dies after surgery. There are six patients to appear pancreatic fistula.There is a patient which occurs serious pancreatic fistula to cause abdominal bleeding and abdominal infection.This patient is treated with antibiotic and gastroduodenal artery embolization.Then,this patient is well. Blood glucose levels of this group patients are stable after surgery.Conclusion:(1) Middle pancreatectomy has important clinical value in the treatment of benign or low-grade malignant tumor in the body and neck of the pancreas.(2) Middle pancreatectomy can reduce damage to surrounding tissues and organs. Middle pancreatectomy is able to maintain pancreatic secretion and digestive function.(3)The most serious complication of middle pancreatectomy is pancreatic fistula. As long as the pancreatic fistula is fund in time,it can be controlled.
