BackgroundThe ear blocked of TCM equals secretory otitis media. The study of the earblocked in these years essentially has two ways :maciosoopic and microcosmic.Acoustic immittance measurement lays special emphasis on the macroscopicdifferentiating syndrome to decide treatment, but western medicine onmicrocosmic differentiating diseases to decide treatment, either of them has itspreponderance. If combination both of them to make the TCM which with eyes onentirety and macroscopical aspect step to microcosmical aspect, objectify andration, and at the same time to make western medicine which with eyes on partand microcosmical aspect to entirety and synthesis, it will being a great progresson theory of differentiating syndrome to decide treatment.A lot of clinic reports showed that pure tone audiometer etc. have somerelationships with the different syndrome of the ear blocked but more of themwere not universality either peculiarity. If use them as the diagnosis criterion ofsyndrome, still needs further clinical test.PurposeTo discussion the correlation between the TCM syndrome of the ear blockedand acoustic immittance measurement.And the the aim to make the results ofwhich can do favour in the macroscopic differentiating syndrome to decidetreatment of the ear blocked.Object and methodAdopted the investigative method to study, selected 65 cases(106ears) whoaccord with the criterion from the ear blocked patients. Recorded the ordinaldatum, TCM syndrome value of the ear blocked, syndrome type, pure toneaudiometer, acoustic immittance measuremeant.Took the acoustic immittancemeasuremeant as dependent variable, had one-way factor analysis with thesex,age,cold history, integral of syndrome, syndrome type of TCM. Used the correlation analysis to analyse intimate degree between acoustic immittancemeasuremeant and TCM syndrome value and also syndrome type of TCM.Usedthe Multinomial logistic regression analysis to analyse the syndrome type of TCMResult1. One-way factor analysis shows that:(1) There is diffence in syndrome type of TCM in 65 cases which broght into:themost is Wind raids,stuffed in ear collaterals, covered 46.2%,the frequency ofDampness and heat of heptic and gallbladder is on intimate terms with Splenicdysfunction in transportation,Qampness surrounds ears’s,and the Evilstagnation,Qi and blood stasis is lest;(2) There is frequency diffence distributed in static compliance、peak pressure、thedegree of tympanogram、the type of tympanogram、acoustic stapedius reflex inacoustic immittance measuremeant;(3) There is obvious diffence in the degree of the incomportable blockedsyndrome between the negative type of acoustic immittance measuremeant andthe normal one;2. Correlation analysis shows that:(1) There is correlation between integral of syndrome and peak pressure、thedegree of tympanogram、the type of tympanogram、acoustic stapedius reflex inacoustic immittance measuremeant,and the correlation coefficient of peakpressure and the type of tympanogram is bigger than the degree oftympanogram and acoustic stapedius reflex’s;(2) There is correlation between syndrome type of TCM and peak pressure、thetype of tympanogram、acoustic stapedius reflex in acoustic immittancemeasuremeant, and the correlation coefficient of peak pressure and the type oftympanogram is bigger than acoustic stapedius reflex’s;(3) There is correlation between integral of syndrome and the distance from airhearing threshold to bone hearing threshold;(4) There is correlation between syndrome type of TCM and the distance from airhearing threshold to bone hearing threshold.3 Took the acoustic immittance measuremeant, as dependent variable, and tookthe syndrome of TCM as independent variable, used the Multinomial Logisticregression analysis to analyse, results showed: There is correlation between thesyndrome type of TCM of the ear blocked and acoustic immittance measuremeant.(1) There is main correlation between normal type of acoustic immittancemeasuremeant and Wind raids,stuffed in ear collaterals,also Dampness and heat ofheptic and gallbladder;(2) There is main correlation between flat type of acoustic immittancemeasuremeant and, Wind raids,stuffed in ear collaterals also Evil stagnation ,Qiand blood stasis;(3) There is main correlation between the negative type of acoustic immittancemeasuremeant and Dampness and heat of heptic and gallbladder ,also Splenicdysfunction in transportation,Qampness surrounds ears’s.ConclusionThere is correlation between the syndrome type of TCM of the ear blockedand acoustic immittance measuremeant, There is correlation between integral ofsyndrome and peak pressure、the degree of tympanogram、the type oftympanogram、acoustic stapedius reflex in acoustic immittance measuremeant.
