Objective:The aim of our study was to assess the value oftwo neutrophil antigens(CD11b and CD64)for early diagnosis of central nervous system(CNS)infection in children.Methods:80cases in the central nervous system(CNS)infection,the Shanxi Province children’s hospital from Sep,2011to Oct,2012were enrolled inour study.40cases of bacterial meningitis were seted as group1,40cases of viral meningitis were seted as group2,20non-infected, healthy children cases were seted as control group. Their blood samples were obtained for CD64and CD11b measurement.blood routin test and cerebrospinal fluid(CSF)test had been done in the labroary of the hospital.The data were dealt with SPSS16.0. and were described in (x±s).P values<0.05was considered to be statistically significant.One way Kruskal-Wallis,Bonferroni,WilcoxonPearson correlation analysis,ROC curve were used for statistical analysis.The sensitivity and specificity of CD64、CD11b were calculated.Results:1. The changes of CD64、CD11b among the groups of bacterial meningitis,viral meningitis and the control.1.1The expression of CD64with bacterial meningitis,viral meningitis and the control were1.882±0.166,1.078±0.048,0.914±0.031. There was significant difference among the three groups. CD64in the group of bacterial meningitis was higher than that of viral meningitis and the control(P<0.05).There was no marked difference between the group of viral meningitis and the control(P=0.303).1.2The expression of CD11b with bacterial meningitis,viral meningitis and the control were25.510±1.788,15.980±0.452、13.380±1.018. There was significant difference among the three groups.CD11b in the group of bacterial meningitis was higher than that of viral meningitis and the control(P<0.05).There was no marked difference between the group of viral meningitis and the control(P=0.124). 2. The changes of CD64and CD11b during the beginning and recovery stage of the group of bacterial meningitis. In the group of bacterial meningitis, the expression of CD11b during the beginning stage was higher than recovery stage distinctly (P<0.01), and there was marked difference between the recovery stage and the control(P>0.05).3. The relation among CD64, CD11b and white blood cell of bacterial meningitis the coefficients between CD64and WBC、CD11b and WBC were0.284、0.293(P>0.05), no liner correlation.4. The calculated optimal cutoff values of CD64,CDllb, derived from the ROC curve, and calculation the sensitivity、specificity.Draw the ROC curve of the levels of CD64、CD11b in diagnosing bacterial meningitis.The area under the ROC curve were0.880、0.853.The best cutoff value of CD64was1.30,the sensitivity was88.3%, the specificity was90.4%.The best cutoff value of CD11b was15.85,the sensitivity was70.4%, the specificity was75.9%.Conclusion:CD64and CDllb levels in creasesignificantly in bacterial meningitis.They may play important roles in the early diagnosis and the differential diagnosis of bacterial meningitis.Test CD64and CDllb dynamicly could observate the efficacy and development of illness.CD64is an ideal diagnostic marker to early identify bacterial meningitis.CD64levels are useful for diagnosis,prognosis or therapy in bacterial meningitis.Its use can decrease mortality and increase survival quality of bacterial meningitis.
