Objective:The observational study was implemented to estimate the relationship between elements of syndrome and volume of acute cerebral ischemic stroke (72-hour onset) and the effect of complications including hypertension, hyperlipidemia and diabetes on the elements of syndrome were discussed.At last, the distribution regularity and transformation of elements of syndrome for three time points (72-hour onset,7-day onset, and14-day onset) was also analyzed.Methods:It was a non-controlled, non-intervention, prospective, multicenter observational study. There were totally three clinical observational centers including dongzhimen hospital, Beijing University of Chinese Medicine, Hebei province Cangzhou hospital of integrated traditional and west medicine and the second hospital of Hebei medical university.112respondents completed the trial which had three time points (72-hour onset,7-day onset, and14-day onset). We collected the elements of syndrome according to the Diagnostic Standard of Syndrome Differentiation of Stroke at every point time, and just recorded the National Institutes of Health Stroke Scale (NIHSS) at two point time (72-hour onset and14-day onset). All imaginings were measured by ITK-SNAP2.4.0, data entry was used by Epidata3.1and the final data was analyzed by SPSS17.0.Results:Overall,78men (69.6%) and34women (30.4%) aged from23-80completed the clinical observation. The number of respondents for anterior circulation stroke (n=81) is much more than posterior circulation stroke (n=31).To analyze the relationship between elements of syndrome and volume of acute cerebral ischemic stroke found that the infarction volume was positively associated with wind, fire and qi-deficiency. Furthermore, anterior circulation stroke was positively associated with wind, fire and qi-deficiency and unilateral anterior circulation stroke was also positively associated with wind, fire and qi-deficiency. However, bilateral anterior circulation stroke was not associated with any element of syndrome. We found that posterior circulation stroke was positively associated with wind, while negatively associated with hyperactivity of yang due to yin-deficiency. Furthermore, unilateral posterior circulation stroke was negatively associated with hyperactivity of yang due to yin-deficiency and positively with wind. Wind and fire were more frequently happened to the patients who combined with hypertension III, phlegm was more frequently happened to the patients who combined with hyperlipidemia and hyperactivity of yang due to yin-deficiency was more frequently happened to the patients who combined with diabetes.Wind was the most frequency element of syndrome, and fire, phlegm, qi-deficiency, stasis, hyperactivity of yang due to yin-deficiency were in turn at72-hour onset; fire was the most frequency element of syndrome, and phlegm, qi-deficiency, hyperactivity of yang due to yin-deficiency, stasis, wind were in turn at7-day onset; phlegm was the most frequency element of syndrome, and fire, qi-deficiency, stasis, hyperactivity of yang due to yin-deficiency, wind were in turn at14-day onset. The element of syndrome often combined with others and3or4elements of syndrome appeared were the most frequency. None or6elements of syndrome did not appear.Wind, fire, phlegm and the number of element of syndrome combined were positively associated with NIHSS at72-hour onset. And other time point, there was no relationship between NIHSS and other parameters.Conclusions:Volume of acute cerebral ischemic stroke was correlated to wind, fire, phlegm and hyperactivity of yang due to yin-deficiency. And the complications including hypertension, hyperlipidemia and diabetes had effect on the elements of syndrome.
