| Objective:To observe the function of Activating Blood and Detoxification Decoction towards treatment effect and quality of life for patients with advanced ovarian cancer. Method:We collect76advanced ovarian cancer patients from Oncologic Outpatient in Guang’anmen Hospital. They have clearly diagnosed pathological disease which has been reassured by distinguished hospitals. According to diagnostic criteria of TCM Syndromes, we use eight principles, Qi and blood, Viscera Differentiations to analyze their clinical materials, and get solo or duolicate syndromes. Sorted by whether having received Chinese medicine or not, we divide them into two groups:38patients are driven into TCM group and other38into control group. Patients in TCM group have received Activating Blood and Detoxification Decoction after initial surgery and chemotherapy, while patients in control group only received surgery and chemotherapy. They are told that they had better fill up a series of questionnaires and forms in order to get a considerable knowledge of their disease, their treatment experience and symptoms. Details are as follows:1. primary materials:birth date, gender, date of initial surgery and chemotherapy.2. Diagnose:clinical diagnose, pathological qualification, FIGO phase, metastatic zones.3. Treatment experience:received chemotherapy, drugs, time and pain.4. Quality of Life scores:Karnofsky scores, FACT-O, scores of TCM syndromes which include tongue and pulse condition.5. Investigation for toxic and side effects:blood and urine routine, hepatic and renal functions, electrocardiograms and so on. Then we analyze the image and pathological materials to make sure whether an ovarian cancer would relapse. So we can calculate Progression Free Survivals and draw out a picture with Survival Curve. We assess the treatment effect considering about Karnofsky scores and TCM Syndromes score. The Activating Blood and Detoxification Decoction includes: astragalus mongholicus, atractylodes, rhizoma dioscoreae, chinaberry fruit, curcuma zedoary, ligusticum wallichii, rhizoma sparganii, peach kernel, cassia twig, herba hedyotis, scorpio. Usage:One dose a day, boiled by water with200ml, divided into two equivalent parts, drunk30minutes after meals per morning and night. Control group only received initial surgery and chemotherapy. Surgery use tumor cytoreductive surgery and2Cycles of chemotherapy, using TP plan: PTX175mg/m2ivgtt dl, plus DDP75mg/m2ivgtt d2; Or TC plan:PTX175mg/m2ivgtt dl, plus CBP AUC=5ivgtt d2. Results:1. Progression Free Survival:The median PFS of TCM group is18.12months while control group is14.4months. Kaplan-Meier Curve shows that TCM therapy can significantly prolong median PFS (P=0.036<0.05).2. Karnofsky scores:Before treatment, there are no difference between groups. But after treatment, TCM group has risen compared with control group (P<0.01).3. Effect assessment of Karnofsky scores:The effect of TCM group is more apparent than control group (P=0.019<0.05).4. TCM syndromes:Before treatment, there are no difference between groups. But after treatment, TCM group has risen compared with control group (P<0.01).5. Effect assessment of TCM Syndromes: The effect of TCM group is more apparent than control group (P=0.048<0.05).6. Single symptoms:TCM group is more significant than control group in the fields of abdominal pain, abdominal distension, fatigue, anorexia, vocal sound, tongue nature (P<0.05). But both groups have no difference in the fields of facial appearance, tongue fur and pulse condition (P>0.05). Results:The Activating Blood and Detoxification Decoction can improve Progression Free Survival and Quality of Life in the patients with advanced ovarian cancer. |
