Abstract/Summary:
Objective Through the analysis of coronary angiography and intravascular ultrasound (IVUS) examination data in patients with acute coronary syndromes, We discusses the function of intravascular ultrasound in the quantitative analysis of intermediate coronary stenosis lesions, plaque nature, guiding further treatment and evaluation of percutaneous coronary intervention (PCI).Method1) According to the clinical symptom,there are32cases hospitalized at department of cardiology in DATONG NO.3hospital from January2010to March2014.Their primary diagnosis is acute coronary syndrome.21patients are male (65.63%).11patients are female (34.37%). Their age vary from32to73years old and average age is (58.1±10.4) years old. CAG evaluate luminal stenosis of each patient’s left main coronary artery (LM), left anterior descending (LAD), left circumflex (LCX) and right coronary artery (RCA).2)When CAG detected lumen stenosis≥70%,these lesions were performed with stent dirently,the lesions with lumen diameter stenosis50-70%(intermediate coronary stenosis),they were undergone IVUS detection to observe the blood vessel wall structure and measure the lumen cross-sectional stenosis in the narrowest point and analyze the plaque nature. PCI were performed to lesions with with indications of stent placement.Other lesions received medication therapy.3) IVUS detection were performed immediately after PCI to evaluate whether stents placement are ideal or with complications.Result1) In32cases, CAG found17lesions of12coronary artery with lumen diameter stenosis>70%, they were performed with17stent directly.2)38lesions with lumen diameter stenosis50-70%(intermediate coronary stenosis) by CAG,33performed IVUS with lesions luminal stenosis>70%and recieved33stents,23single stent,5double stents involved in28patients.3) The lumen cross-sectional stenosis measurements of38intermediate coronary stenosis detected by IVUS are significantly higher than the CAG with significant difference (P<0.01).4) IVUS immediately showed stent placement all ideal without complications after stent placement without coronary artery dissection, haematoma and stent broken.5)Other5lesions involved in4patients without stent indications by IVUS detection received medication therapy.Conclusion1)CAG can underestimated the intermediate coronary stenosis in different degrees.2) IVUS can make a clear distinction between vulnerable and unvulnerable plaques.3) IVUS can accurately select the stenting indication of the intermediate coronary stenosis.4)It can well assess the effect and related complication after coronary stenting, which plays a very important role in intervention treatment of coronary heart disease.
