Adolescence is an important period for the formation depth of sports concept and lifelong physical exercise behavior, this part from the family, school, social integration of Physical Education (hereinafter referred to as "integration") with basic theories, from the sports and health education combined with the needs of the reality, combined with the status quo analysis "integration" of physical education and health education, exploration to put forward the idea of "long-term mechanism for the development of physical and health education integration". In the process of building the model, relates to the knowledge of philosophy, sociology, pedagogy, etc., attempts to give a way of sports health education "integration", and tries to have some innovation, in order to be able to be implemented and verified in the course of practice. This paper mainly research related literature through literature method and expert interview method to collect, compile a list form and the influence factors of youth physical activity, including health education, school physical education scale, family physical education health scale, community sports health scale, adolescent self support sports fitness scale. Through the questionnaire survey method to collect the first-hand data, using SPSS13.0to analyze and process the collected data, the application of AMOS5integrated model. The analysis found that1) the decision support factors of young people themselves according to their contribution in the order of sports theory skills (λ=0.84), participation in sports (λ=0.81), values (λ=0.74), interest attitude (λ=0.68); support factors of family physical education is a long example (λ=0.79), family economic (λ=0.74), parents know (λ=0.61); support of school physical education on Youth Sunshine Sports Factors of the exercise intervention (λ=0.81), health education (λ=0.77), physical education class (λ=0.72), School Physical Education Policy (lambda=0.70), school sports management (λ=0.68), school facilities and funds support (λ=0.67), sports talent (λ=0.61); support factors of social sports education to the Youth Sunshine sports are:community sports organization and management (λ=0.75), community sports venues and funds support (λ=0.68), the community fitness environment (λ=0.61).2) by each dimension contribution that family, school, social responsibilities and tasks in youth sports health education, including "exercise" in the school sports education is the most important,"parents example" is at the core position in the family sports education, society "organization and management" system must be perfect, because it plays a key role in the community physical education.3) from three integrated model structure can be seen between school, family, society the three are closely linked (correlation coefficient is the school and the family0.78, family and community0.71, school and community0.65), from the model we derive a formula:"the intervention of+0.55×family sports health benefits perceived"=0.38×community sports health education intervention +0.64*school physical education intervention, from the formula to highlight the school physical education and health education as the center, take the family as the foundation, the community sports health education as the basis, coordination of the three parts, from the space and time view, the three organic convergence, complementary advantages, fully shows that the family is the initial link of youth sports education, the school is the center link, and the society is extended link.4) From the model a formula (its behavior support=0.63*"perceived benefits") can be drawn from the "main program changes the effect of health education intervention on Sports integration" is the first cause psychological changes (changes in adolescent perceived benefits), psychological change is mainly caused by the behavior change."To build a long-term mechanism of the integration of sports and health education of China’s reform and opening up, explore the reform of school physical education theory, not only inherits the traditional school sports excellent thought, also has the new measures and new ideas in the face of reform and opening up and the various new problems appeared in the process of modernization, realize the new special point taken, it the construction and validation of our theory of socialist modernization education exploration has a positive practical significance and application value.
