Objective To study the protective effect of Milrinone on lung incardiac valve replacement surgery and the prevention and treatment ofacute lung injury after cardiopulmonary bypass and its principles.Methods40patients undergoing valve replacement surgery wererandomly divided into milrinone group (group M) and control group (groupC),20cases each. The patients in group M were given milrinone in aloading dose of50μg/kg before anesthesia induction,followed by acontinuous intravenous infusion of0.5μg/kg/min; and Group C at thesame rate infusion of normal saline. Blood samples were taken from radialartery before anesthesia induction (T1), at10minutes of CPB (T2), andat2hours (T3),6hours (T4) after the operation. Measured the TNF-α,IL-6and IL-10level; Artery blood gas analysis was done at4hoursafter the operation and calculated OI,A-aDO2and RI,Recorded time ofmechanical ventilation and pulmonary artery systolic pressure.Results Compared with T1, TNF-α, IL-6activity increasedsignificantly from T2to T4in both groups,but those levels in groupM were significantly lower in group C; compared with T1, IL-10activityincreased significantly from T2to T4in both groups,but IL-10in groupM were significantly higher than that in group C(P <0.05or P <0.01).OI, group M was significantly higher than group C(P <0.05); A-aDO2,RI,PASP, mechanical ventilation time, group M were significantly lower than group C (P <0.05).Conclusion Milrinone can play a significant role in lungprotection during Cardiopulmonary bypass.
