Objective:To investigate the effect of ulinastatin on liver and kidney for patients of perioperative period in cardiopulmonary bypass during cardiac valve replacement and different administration methods on function of liver and kidney protection effect, to provide a basis for the clinical medication.Methods:75patients were randomly divided into three groups, the first group to ulinastatin (A group):10000U/kg add to cardiopulmonary bypass; second group to ulinastatin (B group):cardiopulmonary bypass at10000U/kg joined the priming solution and on the day of operation to the very next day after operation200000U/time, twice a day, intravenous injection; control group (C group):do not give ulinastatin, three groups of other medication for the same. Draw blood from the vein at the first day before operation> the first day after operation、the third day after operation,to detecte alanine aminotransferase (ALT),aspartateaminotransferase(AST),totalbilirubin(TB),directbilirubin(DB),albumin(ALB), serum urea nitrogen (BUN),creatinine (Cr).Results:ALT、AST、TB、DB、ALB, before operation,these indices have no significant difference (P>0.05), compared the first day after operation with before operation, these indices have a significant difference (P<0.05). At the first day after operation、the third day after operation,compare at the same point in time, these indices have a significant difference (P<0.05), BUN、Cr,before operation these indices have no significant difference (P>0.05), compared the first day after operation with before operation, these indices have a significant difference (P<0.05). At the first day after operation、the third day after operation,compare at the same point in time, these indices have a significant difference (P<0.05).Conclusion:Cardiopulmonary bypass triggers a systemic inflammatory response, and impairs the function of liver and kidney; Ulinastatin can attenuate the systemic inflammatory response induced by cardiopulmonary bypass, and protects the function of liver and kidney; give ulinastatin, at the time of the operation and after the operation, effects of ulinastatin on function of liver and kidney protection effect is better than only given ulinastatin at cardiopulmonary bypass.
