Objective: Exploring the application of3.0T magnetic resonance4DCE-MRA in the diagnosis of Intracranial aneurysm. Comparing of3D-the CTA,4Dthe CE-MRA,3D-DSA three inspection methods in aneurysm diagnosis andmorphological characteristics of significance to assess the three ways used incombination, we can further clarify the corresponding features of intracranialaneurysms, which give us a imaging guidance in the surgical treatment of theaneurysm or embolism. with the development of magnetic resonancetechnology,4D CE-MRA will likely become a more reliable noninvasive methodin aneurysm screening, assessment of preoperative vascular conditions andPostoperative follow-up.Methods: We collect54patients,who are admitted inNingxia Medical University General Hospital neurosurgeon in February2011toApril2012,which are diagnosised clearly of intracranial aneurysm bythree-dimensional CT angiography (3D-CTA), whole brain angiography (3D-DSA)and the four-dimensional contrast-enhanced magnetic resonance angiography (4DCE-MRA Cinegraphy). Through the analysis of the results of aneurysm size,location, shape, number, tumor thrombus, tumorwall calcification and the width ofthe tumor diameter, which are detected by the three different ways,and comparethe differences of three checked way.Results (1) Using3D-DSA,3D-CTA and4Dthe CE-MRA to exam54patients with intracranial aneurysms, we found that57aneurysms and1missed cases by3D-DSA,53aneurysms and5missed cases by3D-CTA and54aneurysms and4missed cases by CE-MRA. The sensitivity and specificity of the three methods were98.27%and95.00%,91.38%and79.17%,93.10%and82.60%;A consistent rate of3D-DSA,3D-CTA compared with4DCE-MRA were0.9298and0.9474; There is not statiscally significant among3checked methods (P>0.05);(2)4D CE-MRA can accurately measure aneurysmsize and neck width, which is statistically significant (P>0.05) in thediagnosis of intra-cranial aneurysm morphological characteristics number of tumor,tumor diameter and neck width, Comparing with3D-DSA and the4D-DSA;(3)The result of4D CE-MRA is closely to that of3D-DSA in intracranial dynamicanalysis of blood flow of the anterior communica-ting artery patency.4DCE-MRA is superior to3D-CTA and3D-DSA examination in endovascularthrombosis. Conclusion: With the further development of magnetic resonancetechnology, the clinical value and scope of application of the4D CE-MRA willcontinue to increase.4D CE-MRA is expected to partially replace invasive DSAexamination in the diagnosis of intracranial aneurysms. with short operated time,safe, non-invasive, low radiation, and no renal toxicity, which is a more reliablenoninvasive method in preoperative vascular condition assessment, earlypreoperative assessment, choice of surgical procedure and postoperativefollow-up.
