| Objective:To describe the symptom experience of patients with liver cirrhosis (frequency,severity, incidence degree), coping strategies and the quality of life,to explore therelationship between coping strategies and experience symptom in these patients, andthe impact of symptom experience on quality of life in these patients.Method:By convenient sampling,125liver cirrhosis inpatients from tertiary A-levelhospitals in Jiangxi were recruited into this study.The Demographic questionnaire,MSAS symptom assessment scale, Medical Coping Modes questionnaire, andChronic Liver Disease Questionnaire were used for the survey.By descriptive analysismethod, t test, anova, Pearson correlation analysis, multiplte linear regression analysisfor data analysis.Result:1. Mainly happened symptoms during the period of hospitalization of patientswith liver cirrhosis included fatigue, abdominal distension, pain, drowsiness, weightloss, loss of appetite, difficulty sleeping, feel sad, dry mouth, anxiety.There were19symptoms having higher incidences(>50%).Patient’s psychological symptoms scorewas1.02士0.51, physical symptoms score was1.03士0.36, the total index of GDIscore was1.13士0.41, the total MSAS score was0.80士0.29, supplemented entriessymptoms score was1.00士0.55.2. The symptom experience in patients with liver cirrhosis were influenced bypayment of medical care,hospitalization time, etiology, course of the disease andgrade of the child.Patients with private expense had a higher level of symptomexperience than others (P <0.05or P <0.01);Patients with more hospitalization timeshad a higher level of symptom experience than patients with less hospitalization times(P <0.05or P <0.01);Viral liver cirrhosis patients had a higher level of symptomexperience than others(P <0.05or P <0.01);The Child C patients had a higher levelof symptom experience than other patients (P <0.05or P <0.01);.Patients with longer course of the disease had a higher level of symptom experience than others (P <0.05or P <0.01).3.Compared with the norm,patients with liver cirrhosis had lower scores in theconfrontation coping strategy than the norm, and had higher scores in avoiding andyielding coping than the norm(P<0.05or P<0.01).The "avoidance" coping strategywas positive correlated with patient’s physical symptoms,the general index andsupplemented entries(r=0.52~0.77, p <0.01).The "yield" coping strategy was positivecorrelated with patient’s psychological symptoms, physical symptoms, the total indexand supplemented entries(r=0.63~0.96, p <0.01).4. The quality of life of patients with liver cirrhosis was negatively correlatedwith their symptom experience.The level of symptom experience was negativelycorrelated with each domain of CLQD(r=0.05~0.86, P <0.05or P<0.01).Conclusion:1. During the period of disease in patients with cirrhosis, the main symptomsinclude fatigue, abdominal distension, pain, drowsiness, weight loss, loss of appetite,poor sleep, feel sad, dry mouth, anxiety.The overall level of symptom experience inpatients is low, but the level of psychological symptoms is high.2. The symptom experience of patients with liver cirrhosis was influenced by thepayment of medical care,hospitalization time, etiology, course of the disease and thegrade of child.Patients with private expense, more hospitalization times, viral livercirrhosis, longer course of the disease and higher Child lever had higher symptomexperience than others.3.The patients with cirrhosis tended to adopt"avoidance"and"yield"copingstrategies and less confrontation strategy.The more "avoidance" and "yield" copingstrategies the patients used,the higher the symptom experience score they had.4.The quality of life of patients with cirrhosis was influenced by symptomexperience.They has negative correlation.The higher symptom experience the patientsexperienced,the poorer the quality of life they had.... |
