OBJECTIVE To evaluate the clinical effects and the curative effects’ influence factor of the Peroral endoscopic myotomy for achalasia.METHODS The clinical data of37cases from October2010to April2014at our center, who were diagnosed Achalasia (AC) and underwent Peroral endoscopic myotomy (POEM) were reviewed. Complications during and after POEM and during follow-up and the curative effects were analyzed.RESULTS All the37patients underwent POEM successfully. The operation time ranged from33min to163min, the length of endoscopic myotomy of inner circular muscle ranged from6cm to12cm, patients were followed up for3-41months, the Eckardt Score before the operation was3-11points(mean7.38±1.769), the Eckardt Score after the operation was0-5points(mean1.86±1.357), lower than before significantly (P<0.05),4cases the Eckardt Score greater than4points after3months of the operation, and one of them underwent the second operation(POEM), and this one was followed up for13months and the Eckardt Score was less than3points. The incidences of complications was21.62%(8/37), no POEM-correlated death occurred. All the complications were cured by conservative treatments. No additional surgery was needed.CONCLUSION POEM is a promising new treatment for achalasia resulting in short-term efficacy. But the long-term efficacy and complications need to be further confirmed.
