objective: The study probes into researching the effects of blood gas analysis inclinical diagnosis and prognosis of massive cerebral infarction in elderly patients,explore the change mechanism of blood gas analysis on massive cerebral infarctioncomplicated with respiratory failure in elderly patients and compare the changebefore and after mechanical ventilation, in order to provide the best treatment basisfor clinical treatment of massive cerebral infarction complicating acid-base balancedisordersMethods: A retrospective analysis of40cases of massive cerebral infarction inelderly patients,admitted to Union Hospital affiliated to Fujian Medical Universityfrom2010to2013is conducted, all cases with basic integrity of the medical records.Glasgow Coma Score is applied.,Peripheral oxygen saturation is moniyored, and dataof arterial blood gas analysis is collected, arterial blood gas analysis and oxygensaturation monitoring changes before and after Endotracheal intubation andmechanical ventilation are recorded. and the prognosis is assessed on the basis ofGOS, thus to investigate the relationship of changes in blood gas analysis andmechanical ventilation and outcome of the disease. Statistical data are analysed withthe application of SPSS.Results: Hypoxemia(Pa02<60mmHg)is an important factor affecting the prognosisof patients with massive cerebral infarction, Compared with the normal oxygenpartial pressure group,the difference was statistically significant(.p<0.05).Acid-baseimbalance(pH<7.35/pH>7.45)seriously affects the prognosis, compared with thenormal group, the difference is significant(p<0.05);Time to Intubation is also animportant factor affecting the prognosis of patients,there are significant differencesbetween the intubation in or after24hours(p<0.05);Mechanical ventilation cansignificantly improve the index of blood gas analysis of patients with massive cerebral infarction,compared with the previous mechanical ventilation, the difference isstatistically significant (p <0.05).Conclusions: Hypoxemia is closely related to the prognosis of elderly patients withmassive cerebral infarction;Also acid-base balance is closely related to the prognosisof elderly patients with massive cerebral infarction too.Early tracheal intubation andmechanical ventilation can improve blood gas analysis and prognosis.
