Endovascular Aortic Aneurysm Repair Versus Open Surgical Repair For Ruptured Abdominal Aortic Aneurysms:a Meta-analysis

Objective By comparing the endovascular aortic aneurysm repair(EVAR) and open surgical repair(OSR) in the ruptured abdominal aortic aneurysms in the treatment curative effect, in order to evaluate the efficacy and safety of endovascular aortic aneurysm repair(EVAR) for ruptured abdominal aortic aneurysm (RAAA).Methods The system retrieves the PubMed database、MEDLINE、EMBASE、Cochrane Library、China Academic Journal、Chinese Biomedical Database、WEIPU Database、Wanfang Database、CNKI Chinese HowNet、Foreign Language Medical biological HowNet，Retrieval time for building adatabase to March2014. Inspection the Reference Supplement included in the study. RevMan5.2software was usded for evaluating methodological quality of included studies. Results A total of9articles (8foreign language and1in Chinese),including2randomized controlled trial (RCT) reports,3for a retrospective case-control study, the remaining4articles for cohort of2937patients,907cases of patients with EVAR,2030cases of patients with OSR.Meta analysis showed: in the rupture of abdominal aortic aneurysm treatment, compared with OSR, EVAR can obviously reduce the30d mortality (OR=0.58,95%CI (0.49,0.69), P <0.00001) and complications (OR=0.58,95%CI (0.50,0.50), P <0.00001).Conclusions The patients with ruptured abdominal aortic aneurysm which the anatomical structure is suitable for EVAR, EVAR has more advantages than conventional OSR, short-term curative effect is better, it can significantly reduce mortality and postoperative complications is, EVAR in the treatment of patiens with RAAA is safe and effective.
