Objective: To summarize the experiences and mid to long-term results of modifiedaortoplasty for the aneurysmal dilatation of the ascending aorta with or withoutvalve disease.Methods: From January2003to October2013,27patients with aneurysmal dilatationof the ascending aorta admitted to our hospital and underwent modified aortoplastywith external wrap were analyzed retrospectively and followed up.Results: The average cardio-pulmonary bypass time of25patients was (130.9±41.4)min(78~269min),and aortic arrest time was(76.3±22.8)min(39~119min).Two patientswere underwent modified aortaplasty without cardio-pulmonary bypass. In no case,postoperative death was causally related to the modified aortoplasty procedure. Twocases had complications during postoperative stage. One case had ischemic stroke,andthe other had Hypotension and arrhythmia. The mean postoperative ascending diameterwas (36.3±3.4)mm (versus preoperative46.4±4.8mm,P<0.05).Twenty four of the27patients were available for follow-up (range1to120.5months).No death wascausally related to Heart event.Two cases died of pneumonia and its complications.Another2patients died of cerebral hemorrhage. One case suffered from uppergastrointestinal bleeding. Aneurysmal redilatation occurred in one patient3yearslater. The mean diameter measured after1year and3years were(40.3±4.3)mm and(40.3±5.6) mm,differed significantly when compared with preoperative andpostoperative diameter(P<0.05).In all cases,23cases had aortic valve disease(3cases were bicuspid aortic valve).Conclusion: Modified aortoplasty with external wrap proves excellent mid to long-termresults and a durable treatment for the aneurysmal dilatation of the ascending aortawith or without valve disease.
