Objective:To restrospectively evaluate the endovascular techniques ofpreserving iliac artery for the concomitant common iliac artery aneurysmduring EVAR, and to summarize the technique and experiences for thepreservation of internal iliac artery.Materials and Methods:From January2010to December2013, a total of42AAA patients treated by EVAR,meeting the inclusion criteria, wasdistributed to study group and control group respectively. All patientswere selsected from the First Affiliated Hospital of Fujian MedicalUniversity. Clinical and morphological characteristics,operativeresult,peri-operative complications and follow-up data were reported. Results: A total of42patients who were treated with EVAR wererestrospectively identified, including study group(n=16) and controlgroup(n=26). The follow-up time was from1month to12months。Thetechnical success between two group were100%.The clinical success ofstudy group was87.5%,including one type Ⅰa endoleak and one iliac limbocclusion, and control study was84.6%(P=0.641).There was no differencein endoleak between study group(25%) and controlgroup(15.4%).Post-operative iliac limb occlusion rates were similarbetween two groups(study group12.5%VS. Contorl group3.8%,P=0.547).There was no significant difference in reinterventions(6.3%VS.15.4%,P=0.633).Conclusion: There are so many techniques to preserve the iliacartery,Bell-bottom technique becomes the first-line alterative for its safe and effective. We advocate its use for common iliac arterysize<25mm.Other endovascular techniques should be use for iliacaneurysm>25mm,such as sandwich technique, iliac branch graft deveces orcrossover technique.
