【Objective】To evaluate the expression of EGFR mutation by Amplification refractory mutation system-polymerase chain reaction (ARMS-PCR) and level of EGFR protein and EGFR mutation specific antibodies by immunohistochemistry (IHC).To investigate their diagnosis effect in lung adenocarcinoma.【Methods】1. The EGFR mutation was detected by ARMS-PCR in288lung adenocarcinoma specimens.50biopsy specimens from normal lung were selected as control at random.2. The lever of EGFR protein and EGFR mutation specific antibodies were detected by EliVisionTM plus immunohistochemical in the same specimens as part1.【Results】1.Of288lung adenocarcinoma specimen,194(67.4%) showed EGFR mutation by ARMS-PCR. EGFR mutation related to gender,smocking history,histological type and TNM(P<0.05).There was no significant association between EGFR mutation and age,tumor size,depth of invasion,lymph node metastases,or distance metastases (P>0.05).2.Positive ratio of EGFR protein was40.6%(117/288), and had significance correlation with lymph node metastases (P<0.05),while not with gender, age, smocking history, tumor size, depth of invasion, histological type, TNM, distance metastases and mutation of EGFR. Del E746-A750antibody was positive in35.8%(103/288) lung adenocarcinoma, and had significance correlation with smocking history and gender,while not with age, tumor size, depth of invasion, histological type, TNM, lymph node metastases,distance metastases.70.9%(73/103) of del E746-A750antibody positive expression was observed the mutation of EGFR19exon del E746-A750,its sensitivity and specificity were67.6%and83.3%respectively. L858R antibody was positive in22.9%(66/288) lung adenocarcinoma, and had significance correlation with smocking history while no significance correlation was found between L858R antibody and gender, age, tumor size, depth of invasion, histological type, TNM, lymph node metastases,distance metastases.75.7(50/66) of L858R positive expression was observed the mutation of EGFR21exon L858R, its sensitivity and specificity were73.5%and92.7%respectively.No relationship was found between EGFR protein and EGFR mutation specific antibodies.【Conclusions】EGFR mutations were more common in Asian,female,non-smoking,and adenocarcinoma.The EGFR protein expression was higher in the group of lymph nodes metastases,which can be used an indicators of prediction of lung adenocarcinoma lymph node metastases.There was high concordance between EGFR mutation specific antibodies and EGFR mutation,which can be a simple inexpensive screening method to evaluate EGFR mutation.The expression of EGFR protein was no correlation with EGFR mutation and EGFR mutation specific antibodies.
