Objective To investigate the immunophenotypic features, the relationshipand clinical significance between CD38with ZAP-70, cytogenetics and otherprognostic factors in chronic lymphocytic leukemia.Methods36patients newly diagnosed with CLL from June2007to Oct-ober2013in the first hospital of Quanzhou affiliated to Fujian Medical Unive-rsity were analyzed retrospectively. Three-color multiparameter flow cytometrydetected the immunophenotypes with monoclonal antibodies, and Four-color m-ultiparameter flow cytometry ZAP-70and CD38expressing in CD5+CD19+cells.Fluorescence in situ hybridization (FISH) tested p53(17p13.1).Results (1) In the36cases with CLL, the positive rate of CD19, CD20,CD22, and CD23in mature B lineage antigen was100%,91.43%,92.86%and77.78%, respectively; the positive rate of CD5was83.33%; the expressing ofCD2, CD3, CD4and CD8were all negative; the positive rate of κ and λ was57.14%and42.86%, respectively; the positive rate of CD38was27.27%, otherantigens could be detected at different levels but CD10and CD34were all ne-gative.(2) the expression of CD38in CLL were not correlated with sex, age,Binet stage, the lymphocyte count in peripheral blood, and the number of leuk-emia cells in bone marrow except the serum levels of lactate dehydrogenase.(3)The positive rate of ZAP-70and CD38expressed by CD5+CD19+cells was(0.30±0.69)%and (10.74±16.67)%, respectively; Correlation was not found bet-ween them(r=-0.261,P=0.368).(4)The PFS of CD38+and CD38-cases with CLLby Kaplan-Meier survival analysis was14months and27months,respectively; the median PFS of this two groups had statistically significant differences (P=0.027) in log-rank test.(5) FISH analysis showed that p53(17p13.1) was normalin all6cases.Conclusion Immunophenotyping plays an important role in the diagnosisand differential diagnosis of CLL, the high expression of CD38could be as apoor prognostic factor in CLL.
