Objective: Recurrent spontaneous abortion (RSA) refers to the continuous3times or more than3times of spontaneous abortion. About40%of recurrentspontaneous abortion can’t find the causes, this kind of abortion is named asunexplained recurrent miscarriage (URM).So far, we mostly studies women factors tofind the causes of recurrent miscarriage, such as the abnormal genital anatomy,endocrine disorders, immune disorders, hemoglutination disfuction, or reproductivetract infections, and the study about the male factors are quite little.But the sperm as patrilineal genetic carrier in the reproduction, which not onlyprovides a fertilized egg with half of the genetic material, but also plays a crucial rolefor embryonic development.With the deepening of the research on etiology ofrecurrent spontaneous abortion, more and more researchers have focused on spermDNA integrity to explore the causes of unexplained recurrent miscarriage.In addition,the scholars found that oxidative stress is the main pathogenesis mechanism of spermDNA damage, but at the moment antioxidant treatment of unexplained recurrentmiscarriage is still in exploration. Therefore, the purpose of this study was to detect sperm DNA damage by the spermchromatin spread (sperm chromatin dispersion, SCD) method, analyze whether itassociated with unexplained recurrent miscarriage, and to give tentative antioxidanttreatment to the men with the abnormal rate of sperm DNA fragments in unexplainedrecurrent miscarriageMethod:(1) By elected standard, the research object is divided into unexplainedrecurrent miscarriage male group (URM group, n=152),, and normal reproductivemale group (the control group, n=52),collect male semen samples of two groups;(2)compare the two groups on general conditions, such as age,smoking, drinking, andsemen routine parameters—sperm density, sperm vigor, sperm deformity rate;(3)detect sperm DNA fractionation index (DFI) of two groups with SCD method;(4) inrecurrent abortion group,give vitamin C (100mg, three times/d)+vitamin E (100mg,2times/d) antioxidant treatment for3months to men with abnormal sperm DNAfractionation index.Statistical analysis was performed using the SPSS17.0software.measuring data by using t test, counting data by chi-square test, P values of<0.05were consedered significant.Results: Statistical analysis of URM group and the control group:(1) comparesamples of two groups on the general conditions,such as age, smoking, drinkingalcohol, there was no statistical difference (P>0.05);(2) comparing samples of twogroups on semen routine parameters such as sperm density (106/ml), energy (%),deformity rate (%),they were respectively (138.5±20.7vs.145.2±18.5),(50.0±12.9vs.51.2±9.4),(72.1±19.2vs.73.8±17.6) and all had no statistical difference (P>0.05);(3) For two groups, the people with abnormal sperm DNA fractionation index wasrespectively40and4,the abnormal rate is26.3%and7.7%,respectively, there werestatistically significant (P <0.05).The abnormal patients in URM group were randomlydivided into two group—antioxidant treatment group (n=20) and the control group(n=20), reviewed DFI, the recovery rate of abnormal DFI were70%and40%respectively, there were no statistical difference (P>0.05).Conclusion: There was a significant correlation between sperm DNA integrity withunexplained recurrent miscarriage, and there were no correlation between semenroutine parameters such as sperm density, vigor, deformity rate with unexplained recurrent miscarriage. By oral vitamin C, vitamin E,sperm quality of male patients ofunexplained recurrent miscarriage may be improved.
