Objective: Hyponatremia is the most common complication of pituitary adenomapostoperative.It is clinically divided into four types: hypopituitarism hyponatremia,Diabetes Insipidus (DI), antidiuretic hormone secretion disorder syndrome (SIADHS)and cerebral salt consumption syndrome (CSWS);so fatal is hypopituitarismhyponatremia, we can use hormone replacement therapy after diagnosing clearly.But itsclinical specificity is poor, often not typical, that brings to the early diagnosisdifficult,.This research through the hyponatremia of hypopituitarism related factorsanalysis, in order to improve the accuracy of early diagnosis and can give timely andeffective treatment.Methods: Collecting clinically relevant data from90patients hospitalized withpituitary adenoma since January2008to January2014,we analysis of patients’ gender,age, size of tumors and operative methods for the influence of the incidence ofhyponatremia in hypopituitarism.All patients choose to do pituitary adenoma resectionor invasive pituitary adenoma resection surgery pa, and preoperative imaging diagnosisand postoperative pathological diagnosis are pituitary adenoma;All patients in thepreoperative and postoperative routine of pituitary hormone and serum sodiumconcentration check, for patients with hormone levels and low serum sodiumconcentration before the operation have been adjusted to the normal range; All materialsadopt to SPSS2l.0software, using chi-square test, seting to P <0.05with statisticalsignificance.Results: From clinical data on90cases of hyponatremia after pituitary adenomapostoperative,we find that male40is cases, female50is cases;16patients with20-40years old,39patients with40to60years old, more than35patients with60years of age, average age57.4±5.8;62patients choose to sphenoid approach of pituitaryadenoma resection, and other28cases of patients are via to pituitary adenoma resectioncraniotomy;Serum sodium concentrations in patients with a minimum tendency of107mmol/L, highest tendency for134mmol/L. Appearing for postoperativehyponatremia times are from24hours to10days.28cases (31.1%) patients are diagnosis of hyponatremia due to hypopituitarism.10cases (33.3%) are male,and18cases(52.3%) are female.Under the age of60,11casesare in low pituitary function hyponatremia, accounting for20.0%;over60years old, of17cases are hyponatremia due to low pituitary function,accounting for48.6%.Throughadjustment into the road, including13cases of patients with pituitary adenoma resectionsurgery, accounted for20.9%;Line of15patients with pituitary adenoma resectionsurgery craniotomy, accounting for53.5%.Huge glandular pituitary tumor, largeadenoma, micro adenoma postoperative pituitary function with low hyponatremia arerespectively3cases,12cases,13cases, and the incidence acount for respectively,32.5%,70.5%and60.5%.Chi-square analysis of gender has no statistical significance, namely, gender is nothyponatremia influence factors of pituitary function impairment;Age, operation method,and the size of the pituitary adenoma impact factors of hyponatremia due tohypopituitarism.Conclusion:1.Age influence the incidence of hyponatremia due to hypopituitarism. High prevalence are elderly patients not young patients.2.Gender in hyponatremia due to hypopituitarism following surgery of pituitary adenoma has no statistical significance;3.Operation method and the size of the tumors also impact the morbidity of hyponatremia due to hypopituitarism.Prevalence is lower in transsphenoidal m-icrosurgery patients4.If elderly patients develop hyponatremia after nvasive pituitary adenoma resection,we highly suspected of hypopituitarism.
