| Background:Tetralogy of Fallot (TOF) is one of the most common congenital heart disease, imaging examination is particularly important in the diagnosis, cardiovascular imaging (angiocardiography, ACG) has been considered the "gold standard" diagnosis of congenital heart disease, but which is not easy to get children patients to cooperate, at the same time contrast agents is more, children appear more adverse reactions, and the time of imaging is long, the radiation damage is serious, widely used in display anatomy of the heart structure and hemodynamic information, plays an important role in diagnosis of congenital heart disease, Because of its less imaging contrast agent usage, safe noninvasive and scanning speed, scanning range, multislice CT angiography (MSCTA) has been widely used in the diagnosis of congenital heart disease. Objective:To study the value of16layers spiral CT and echocardiography were used respectively to quantitative and qualitative measurement of tetralogy of Fallot preoperative diagnosis, further to discusse the application accuracy of heart disease both in the diagnosis of children’s congenital tetralogy of Fallot, and to improve the effect of tetralogy of Fallot preoperative imaging diagnosis.Methods:38cases0-8years old neonates and infants with tetralogy of Fallot with16layers spiral CT and heart echocardiography data were retrospectively analyzed nearly three years.Using GE Lightspeed16row helical CT for38cases of tetralogy of Fallot with multi-slice CT angiography, Then analyzed, diagnosed and measured images data reconstruction in work station ADW4.2by all kinds of refomation methods and calculated MeGoon Index and Nakata Index. Using Philips Sonos4500type color Doppler ultrasound instrument for children with echocardiography, measured inner diameter of right ventricular outflow tract, pulmonary artery, and calculated the McGoon index and index Nakata. Application SPSS17.0statistical software for data from single factor variance analysis. Results:Four major malformations of tetralogy of Fallot could be accurately diagnosed by both16layers spiral CT and echocardiography. Compared with that of surgery results, there were no significant statistically differences in evaluating of the ventricular septal defect size, aortic straddles, right ventricular outflow tract stenosis and the diameter of main pulmonary artery (P>0.05). Cardiac malformation in echocardiography diagnosis was more clearly than that of16-slice spiral CT, but16-slice spiral CT had more advantages than echocardiography in diagnosing left and right pulmonary artery diameter measurement, MeGoon index and Nakata index of measuring (P<0.05).Conclusions:Echocardiography and16layers spiral CT are better diagnostic significance of tetralogy of Fallot,16layers spiral CT has a noninvasive, fast examination technology and powerful post-processing function, and can accurately diagnosing tetralogy of Fallot merger other heart malformations, especially pulmonary artery, the evaluation of pulmonary artery development condition for a period of radical surgery have an important guiding significance. |
