Clinical Study On The Relationship Between Serum Brain Natriuretic Peptide, Uric Acid, High Sensitive C-reactive Protein And Cardio-renal Function In Patients With Chronic Heart Failure

Objective To study the relationship between the level of serum brain natriuretic peptide(BNP), Uric acid(UA),High sensitive C-reactive protein(hs-CRP) and the damage they caused to cardio-renal function by detect serum BNP,UA,hs-CRP and Left ventricular ejection fraction (LVEF),Blood urea nitrogen(BUN) and creatinine(Cr) of the patients with chronic heart failure.Methods We selected186patients with CHF as the experimental group,they were divided into three groups according to the NYHA class.41patients ranging in class Ⅱ,85in class Ⅲ,60in class Ⅳ.Fifry subjects without CHF were enrolled into group which matched the gender ratio and age range of the experimental groups,to detect the level of serum brain natriuretic peptide(BNP), Uric acid(UA), High sensitive C-reactive protein(hs-CRP), Blood urea nitrogen(BUN) and creatinine(Cr). Patients underwent echocardiography Dopler examinations Left ventricular ejection fraction (LVEF). We try to observe the correlation of serum BNP, UA, hs-CRP, BUN and Cr, and analyses their varies. In this study we divide the objects into three groups, one with a creatinine(Cr) level less than133mmol/L,133~177mmol/L, and one with a level exceeding177mmol/L. At the same time, divide into two groups with blood urea nitrogen(BUN) level in7.0mmo l/L.Results1. Compared with a basic clinical information such as age, gender, the experimental group had no statistical significantce compared with control group.(P>0.05).2. The CHF group have higher level in serum BNP, UA, hs-CRP, comparing with the control group (P<0.05).With the deterioration of cardiac function,LVEF decreased, the levels of serum BNP, UA, hs-CRP increased gradually.There is a negative correlation between BNP and LVEF (r=-0.913, P<0.05). Serum UA level was negatively correlated with LVEF (r=-0.595,P<0.05),and hs-CRP level also showed a negative correlation with LVEF (r=-0.817, P<0.05).3. With NYHA grade raising, BUN and Cr are also gradually increased, BUN and Cr was negatively correlated with LVEF,statistics have significant differences(P<0.05).4. According to cardiac function (NYHA II level, NYHA III level, NYHA IV level), renal function (Cr<133mmol/L group,133-177mmol/L group, Cr>177mmol/L group) Comprehensive classification, the sample was divided into18subgroups. The results show that in NYHA class and renal functional classification of double standards changes of BNP and UA:UA, BNP concentration varies with NYHA grade level not only happen, but also affected kidney function classification{P<0.01), in the same renal function level, UA, BNP increases with increasing NYHA classification (P<0.01); in the same NYHA class, with the kidney function getting worse, the serum BNP, UA levels also appeared a gradual increase significantly (P<0.05), and in patients with more severe NYHA class, this trend becomes more obvious.Conclusions1. The cardiac function has correlation with the serum level of BNP,UA and hs-CRP,withing the cardiac function contaminated, the plasma level of them increases gradually. The BNP, UA and hs-CRP level was positive correlation with NYHA, and negative correlation with LVEF.2. Serum BNP and UA levels have correlation with renal function, and with the Serum BNP and UA levels increasing, the degree of impaired renal function aggravated. Cardiac function and renal function depend on each other, the cardiac dysfunction increase is often accompanied by renal function decrease. In the group of the same level of heart function, patients with impaired renal function, BUN and Cr levels were significantly increased.3. The United plasma BNP, UA and hs-CRP levels and renal function tests can more accurately assess the condition and prognosis of patients with heart failure, it has important significance for guiding us to a comprehensive analysis of the patient’s disease.
