| Objective:To analysis of the etiology and managements of complications after endovascular aortic repair (EVAR). To investigate the treatment of complications and preventive measures, in order to reduce abdominal aortic aneurysm cavity repair of postoperative complicationsMethod:A retrospective analysis of complications of95EVAR cases from2011.1to2014.1Results:All patients finished EVAR smoothly, there are9cases happened related complications after surgery. The complication rate was9%and the main complications were internal leakage5cases(4.75%), intra-stent thrombosis2cases(1.9%), stent migration1case(1%) and intestinal ischemia1case(1%). Patients with internal leakage occurs, intraoperative found the proximal internal leakage type I give a balloon expansion, distal internal leakage type I give corresponding stenting. Stent thrombosis patients give positive thrombolysis treatment, patients with distal stent displacement to coated stents, to isolate tumor cavity. Intestinal ischemia in patients with conservative treatment. Circulatory failure except1case of stent thrombosis patients died, the rest of the prognosis of patients with complications is good.Conclusion:EVAR is characterized by mild tissue injury in the operation, the less blood loss and less pain, the fast postoperative recovery and lower post-operative morbidity.It is the ideal choice for frail elderly suffered by abdominal aortic aneurysm accompanyed many complications. EVAR is effective for patients in the near term. Endovascular technique itself exists stent related complications such as internal leakage, intra-stent thrombosis, stent migration and intestinal ischemia. The key points are precise appraisal before operation, selecting the suitable stent and appropriate choice of operation method.Also, it is necessary to leave the internal iliac artery intact and recheck regularly after operation. So,the complications can be found so as to be treated promptly and efficiently and we can raise the curative effect. |
