Objective:To explore the mechanism of proteinuria and renal protection of prednisone by observing the change of blood and urine biochemical markers,and the expression of Ezrin and NEPH1in rats with adriamycin-induced nephrosis.Methods:Feeding male Wistar rats for a week.All the rats were randomly divided into three groups:blank control group (group A), model group (group B), prednisone treatment group (group C),Group B and C used to make ADR model by a tail intravenous injection of ADR for2times.The first injection of4mg/kg, after1week, the second of3.5mg/kg.The rats of group C was treated prednisone per day.Tp、Alb、 Glo、TCho、Urea、Scr、24h-up were measured at4and8weeks. At the end of the8th week.Measuring glomerular area and glomerular extracellular matrix area.ExtraCellu-Lar matrix/glomerulararea ratio and renal pathology points were determined. The expressions of Ezrin and NEPH1were detected by immunohistochemistry method,and the mean absorbance value was measured and calculated by Image Analysis Software.Use Spss19.0for statistical analysis.Results:1.The changes of various biochemical index of the three groups①roup A:24h-up, TCho, Scr, Alb, Tp, Ccr are normal②Group B:24h-up, TCho, Scr were higher than group A (P<0.05), Alb, Tp, Ccr were lower than group A(P<0.01), at the end of4weeks.Compared with the numerical at the end of4weeks,24h-up, TCho, Scr were significantly higher,Alb, Tp, Ccr were significantly lower than at the end of8weeks.③Group C:24h-up, TCho, Scr were higher than group A (P<0.05), Alb, Tp, Ccr were lower than group A(P<0.01), at the end of4weeks,24h-up, TCho, Scr were significantly lower than group B(P<0.01),Alb, Tp, Ccr were significantly higher than group A (P<0.01);24h-up, TCho, Scr were lower(P<0.01),Alb, Tp, Ccr were higher (P<0.01) at the end of8weeks than at the end of4weeks.2. Changes of renal pathology:①Light microscopic observation: Group A:the structure of glomerular and renal tubular has no obvious change.Group B:part of glomerular appeared balloon adhesions and visceral epithelial cell swelling, mesangial cells and matrix appeared mild hyperplasia,the renal tubules tubule epithelial cell vacuoles degeneration, tubular lumen appeared lot of protein, renal interstitial infiltratiea lot of inflammatory cells.Group C:Lesions are improved obviously,mesangial cells appeared mild hyperplasia,tubular lumen appeared little of protein, renal interstitial infiltratiea little of inflammatory cells②ECM/GA:Compared with groupA, ECM/GA in group B and C was increased(P<0.01). Compared with group B, ECM/GA in group C was decreased(P<0.01)③Renal pathological points: renal pathological points in group B and C were increased(P<0.01).renal pathological points in group C were decreased(P<0.01).3.The immunohistochemical results: Group A:Ezrin and NEPH1present a large number of expression in glomeruli, along the capillaries and uniform linear distribution. Group B:There is a little epression in glomerular, the brown sediments become thin even broke or disappeared, compared with the group A,the epression was significantly decreased (P<0.01).Group C: Compared with group B, The epression of Ezrin and NEPH1increased significantly, with significant difference (P<0.01).4.The correlation analysis:The expression of Ezrin in group B was negatively correlated with the8th week’s24h UP(r=-0.838), TCho (r=-0.936),Scr(r=-0.956), glomerular pathology point(r=-0.949),ECM/GA (r=-0.972).and positively correlated with Alb (r=0.895),Tp(r=0.994),Ccr(r=0.892), NEPH1(r=0.980). The expression of NEPH1in group B was negatively correlated with the8th week’s24h UP (r=-0.884), TCho(r=-0.952),Scr(r=-0.959),glomerular pathology point(r=-0.971),ECM/GA (r=-0.976).and positively correlated with Alb (r=0.898),Tp(r=0.968)、Ccr(r=0.938).Conclusions:1.The decline expression of Ezrin and NEPH1in rats with ADR-induced nephrosis may be one of mechanism of proteinuria,can also be one of the important factors of nephrotic syndrome.2.Ezrin and NEPH1expression in renal tissues synchronization, Witch prompts Ezrin and NEPH1may be closely related on structure and function.3.Prednisone can reduce the proteinuria and relieve the renal pathological damage by improving the expression of Ezrin and NEPH1.
