Part Ⅰ Clinical ObservationObjectiveQuantitative observation and therapeutic evaluation on the lesions subsiding of psoriasis vulgaris with the treatment of Chinese herbal compound Psoriasis No.1.Methods1. To divide the36patients with psoriasis vulgaris into treatment group and control group randomly. The treatment group took Psoriasis No.1and tripterygium glycosides tablet orally while the control group took only tripterygium glycosides tablet orally. Both groups took Purple Dragon Cream externally. The treatment lasts for four weeks.2. Clinical Observation:To evaluate and give a PASI mark to the pruritus, lesion area and lesions subsiding at the beginning and after each week.ResultsThe cure, the total effective rates were11.1%,94.4%, the cure rate of the control group, the total effective rate of5.56%and83.3%.Shown from T-testing, the pruritus score and PASI of both groups after treatment declined dramatically(P<0.05). There was a significant difference after comparing between the two groups. Comparing the therapeutic effect of the two groups, the total effective rate of treatment group was superior to control group significantly(P<0.05). It had statistically significance. There was no adverse reaction in treatment group but there were two patients having mild nausea in control group, which had been improved after corresponding treatment. No abnormal changes were shown from the routine test of blood, urine, stool and hepatorenal function, chest radiograph and electrocardiogram.Conclusion The use of tripterygium glycosides tablet together with Psoriasis No.1is more effective than the exclusive use of tripterygium glycosides tablet in decreasing the skin lesion of patients with psoriasis vulgaris. The clinical effect is better and there are less side effects.PartⅡ Experimental ResearchObjectiveBy adopting the method of metabonomics, to study the effect of Chinese Herbal compound on HaCaT Cells’ metabolism and related molecular mechanism From the level of histology. To provide potent basis for the study on the function target of Chinese herbal psoriasis No.1and the discussion of molecular mechanism.MethodsTo prepare the cell lysates and supernate of HaCaT cells to carry out the nuclear magnetic resonance (NMR) test. To prepare6cases of normal saline group,6cases of Chinese herbal serum group and6cases of tripterygium group. To determine metabolite fingerprint by means of1H-NMR. To sum up a comprehensive analysis result by applying statistical approaches like principal component analysis(PCA) and discriminant analysis combining with bioinformatics. To discuss the features of target function and the metabolic phenotype of Chinese herbal Psoriasis No.1on HaCaT cells.Results1H-NMR result showed:Metabolic effects of Psoriasis No.1on HaCaT were different from that of normal saline or tripterygium glycosides. There were differences between the total metabolite product of correlation effect. Compared with cell lysates of normal saline, the concentration of hydroxy phenylacetic acid, cholesterol and deoxyinosine in the group with Psoriasis No.1reduced while the concentration of holine glycerophosphatide, iodinated tyrosine,7-dehydrocholesterol and aldosterone increased. Compared with supernate of normal saline, the content of desoxycortone and1-carnitine in the group with Psoriasis No.1changed. Among the materials showed in the result, those who made much greater contribution among the groups were deoxyinosine, cholesterol and7-dehydrocholesterol.Conclusion Psoriasis No.1exerts an influence on the metabolic process of HaCaT. During the process of inhibition of cell proliferation, metabolite products with differences were detected, whose metabolic pathways involved lipids, protein, amino acid, pyrimidine, purine and so on. It dedicates the multiple targets effects of Chinese herbal compound and its regulating effect on the metabolic process inside cells. Psoriasis No.1may influence the formation of fiber cell proliferation of skin with psoriasis through the influence on nucleotide metabolism of epidermic cells and the participation in the regulation of vitamin D metabolic process of epidermis. This might be the mechanism of Psoriasis No.1on treating psoriasis vulgaris.
