Analysis Of Risk Factors Affecting Mortality In Elderly Patients With Femoral Intertrochanteric Fracture

Abstract/Summary:
ObjectiveTo analyze the factors influencing the mortality of elder with intertro-chanteric fracture so as to establish the theoretic basis and provide a guide for a further improvement of curative effect.MethodsFrom March2012to March2013, we have treated174patients of intertrochanteric fracture aging more than60years old in our hospital. Among them,152patients of intertrochanteric fracture were included and followed-up one year. Among the152patients, The clinical data were collected for statistical analysis in order to analyze the factors affecting the mortality of these patients. The factors include ages, sex, type of fracture, blood albumin, hemoglobin, number of co-exiting diseases before injury, ASA classi-fication and POSSUM physiological scores, duration until surgery, type of operation, type of anesthesia, duration of operation, use of transfused blood products, treatment of osteoporosis, post-operative complications. All the predictive risk factors were dealt with Χ2test or t-test, and then multivariate Logistic regression analysis will be used for the statistically significant variables.ResultsThe factors which have significant influence on all intertrochanteric elderly patients on the outcome of the one-year mortality (with p<0.05) include the ages, sex, ASA classification, POSSUM physiological scores, blood albumin, hemoglobin, number of co-exiting diseases before injury, pulmonary disease, cardiac disease, type of therapy, postoperative complications such as pulmon-ary complication, cardiac complication and digestive complication. The multivariate Logistic regression analysis shows that the independent risk factors are pulmonary complication and cardiac complication after injury.The operation paitents:ages, hemoglobin, ASA classification, POSSUM physiological scores, number of co-exiting diseases before injury, pulmonary disease, cardiac disease, post-operative complications such as pulmonary complication and cardiac complication. The multivariate Logistic regression analysis shows that the independent risk factors are pulmonary complication and cardiac complication after operation.ConelusionPulmonary complication and cardiac complication after injury are independent risk factors for intertrochanteric fracture elder patients. Blood albumin, hemoglobin, ASA classification and POSSUM physiological scores, number of co-exiting diseases before injury, pulmonary disease, cardiac disease, type of therapy have effect on mortality.The result suggest that in order to reduce the mortality of intertrochan-teric fractures elder patients, we should enhance understanding of medical conditions and living skills among them, and thus evaluate general condition adequately of the patients, and improve the nutritional status of patients and medical conditions prevent the cardiovascular and respiratory complications.
