The Curative Effect Of Jieduxingnao Fluid For Early Hepatic Encephalopathy Related To Liver Cirrhosis And The Effects Of. Jieduxingnao Fluid On The BA.LPS.TNF-α

Objective:Observation.the.clinical.curative.effect.of.the.treatment.of.early.hepatic.encephalop-athy related to liver cirrhosis with Jieduxingnao fluid and the effects of Jieduxingnaofluid on the BA、LPS and TNF-a and Observation the improvement time of shortenthe clinical symptoms and signs(consciousness, intelligence and behavior change,theflapping wing tremor,abnormal neurological signs) through the experiment.Furtherstudy of the mechanism of action of Jieduxingnao fluid for hepatic encephalopathy byrelieving heat, eliminating phlegm to begin to understand, detoxification, clearingintestinal endotoxin.Methods:(1)According to the hepatic encephalopathy (phase I and phase II) diagnostic criteria,the60caseswith HE were randomly divided into a treatment group30cases and control group30cases.(2)Treatment group30cases were administrated150mL of chinese medicine Jieduxingnao fluidin the form of enema2times a day. Chinese medicine Jieduxingnao fluid will be keep at35to40℃.Enema intubation was inserted into the anus20cm～30cm,and we should drip the liquidslowly, keep the drug in intestinefor more than1hour, with7days in a course.(3)Control group30cases were administrated lactulose cooperation with foundation treatment inthe form of enema2times a day, lactulose45ml was added to100ml saline and the fluid will bekeep at35to40℃.Enema intubation was inserted into the anus20cm～30cm,and we should dripthe liquid slowly, keep the drug in intestinefor more than1hour, with7days in a course.(4)Observe the following:1. Changes in consciousness, intelligence and behavior, the disappeartime of asterixis;2.Change of blood ammonia concentration (before treatment,24h、48h、72h and1week after the treatment);3.Concentration changes of serum endotoxin (LPS)(before and aftertreatment), the concentration changes of the serum tumor necrosis factor alpha (TNF alpha)(before and after treatment);4.Concentration changes of serum alanine aminotransferase (ALT)(before andafter treatment) and concentration changes of total bilirubin (TBIL),(before and after treatment).Results:(1)Baseline of two groups of patients: gender、age distribution、with or without ascites、withor without liver palms、with or without cause of diet、liver disease staging、liver cirrhosisbackdround、prothrombin time(PT)、albumin concentrations (ALB)、the width of portal vein(hereinafter referred to as: the width of portal vein)、spleen size and distribution of plateletcount(PLT) were no statistical significance(P>0.05).(2)Compared with treatment group,the consciousness、intelligence、behavior change and thedisappear time of asterixis of control group were no statistically significant difference (P>0.05).(3)the groups of patients after treatment, the treatment group total effective rate was higher thanthe control group the clinical curative effect compared with the difference was statistically significant(P <0.05)(4)After treatment，concentrations of blood ammonia of two groups were significantly lower (P<0.05);Compared with control group, the curative effect of the treatment group was better (P <0.05).(5)After treatment，concentrations of LPS and TNF alpha of two groups were significantlylower (P <0.05);Compared with control group, the curative effect of the treatment group was better(P <0.05).(6)After treatment，concentrations of ALT, TBIL of two groups were significantly lower (P <0.05);Compared with control group, the curative effect of the treatment group was better (P <0.05).Conclusion:1.The clinical curative effect showed that Chinese medicine Jieduxingnao fluid cooperation withfoundation.treatment.can.rapidly.improve.consciousness.cognition,directionalforce,memory,computi-ng power,abnormal behavior and asterixis in patients with hepatic encephalo-pathy.2.Traditional Chinese Medicine(TCM)Jieduxingnao fluid cooperation with foundation treatmentcan significantly reduce serum concentration of LPS and TNF-a, and the clinical curative effect isbetter than that of lactulose.3.TCM Jieduxingnao fluid cooperation with foundation treatment can significantly reduce theconcentration of serum ALT, TBIL, and the clinical curative effect is better than that of lactulose.
