Objective:The purpose of this study was to investigate the influence of dydrogesterone administration on follicular size discrepancies in in vitro fertilization and embryo transfer(IVF-ET) in low responders when luteal dydrogesterone administration was used in GnRH antagonist protocol.Methods:Altogether191patients undergoing IVF were prospectively randomized into dydrogesterone administration-pretreated group (A group, n=73) and non-pretreated group as control group(B group, n=118).Primary outcome measure was magnitude of size discrepancy of growing follicles. Secondary outcomes were follicular stimulating hormone (FSH), luteinizing hormone (LH),estrogen (E2) on D2, Gn duration and dosage, antagonist duration and dosage, E2,progesterone(P),LH and endometrial thickness on HCG day, number of oocytes retrieved, high-quality embryo rate, implantation rate, clinical pregnancy rate and ongoing pregnancy rate were compared in the two groups.Results:The level of FSH on D2(7.75±4.03vs.9.28±4.08,P=0.012) was decreased in group A. the level of E2(4,593.39±2,334.35vs.3,784.94±2,065.36P=0.013) on HCG day and the number of oocytes retrieved (5.29±3.03vs.4.31±3.18,P=0.036) were increased, follicular size discrepancies on the7th day of Gn (16.44%vs.31.36%,P=0.022) and on the day of HCG injection (23.29%vs.37.29%, P=0.044)attenuated in the group A compared with the control group.Conclusions:Luteal dydrogesterone administration can improve size homogeneity of growing ovarian follicles.
