The Clinical Research Of Carotid Endarterectomy Treatment Of Ischemic Cerebrovascular Disease Caused By Extracranial Carotid Stenosis

Research purpose:This article focuses on the hospital medical records of396patients with severe internal carotid artery stenosis, carotid endarterectomy is studiedand discussed in treatment of carotid stenosis extracranial safety and effectiveness ofischemic cerebrovascular disease.Research methods: Dalian municipal central hospital were retrospectivelyanalyzed in March2009to August2013, diagnosed and treated396cases of patientswith severe internal carotid artery stenosis, including200cases of internal carotidendarterectomy surgery treatment,196cases by the medicine conservative treatment,induction, comparison with the result of medical records and follow-up, the summary,and the data were statistically analyzed. Through the classic example of comprehensiveanalysis, in this paper, the internal carotid artery intima stripped carotid stenosisextracranial treatment caused by the safety and efficacy of ischemic cerebrovasculardisease.Results: patients with preoperative and postoperative clinical symptoms weresignificantly improved in178patients,1case of patients after cerebral infarction,11patients with clinical symptoms without change;10cases of preoperative patients withclinical symptoms were not new clinical symptoms after surgery. All of the patientshave no perioperative death. Follow-up of200cases, with an average follow-up of32.5months (20to45months), all patients were not present new stroke and other seriouscomplications. Medical treatment group in20months observation period new cerebralinfarction47cases,6cases of large area cerebral infarction, cerebral hemorrhage4 cases,1case death, TIA79cases of recurrent carotid artery occlusion in2cases,gastrointestinal bleeding in1case,18cases of abnormal liver function, muscle pain,1case. Clinical therapeutic effects of two groups of comparison, the scores of the clinicaltreatment of total effective rate significantly higher than that of medical treatmentgroup.Conclusion: Extracranial carotid endarterectomy is the treatment of carotidstenosis ischemic cerebrovascular disease is a safe and effective treatment, the clinicaltreatment effect is better than medicine treatment group. After taking strict control ofblood pressure, TCD monitoring method can reduce and prevent postoperativecomplications of carotid endarterectomy.
