Objective(1) To evaluate the etiology,course of disease,the clinical features and treatment status of corticosteroid-dependent dermatitis, providing the basis for its diagnosis and treatment.(2) To observe the effectiveness and security of the clinical efficacy on corticosteroid-dependent dermatitis with Intensive Moisturizing Cream containing prinsepia utilis royle oil, purslane extract.Methods(1) Clinical analysis:The questionnaire survey method used by the9Hospital Department of Dermatology from January1,2013--October31,2013.The patients were required to fill in the questionnaires designed according to the general conditions,etiology,course of disease,the clinical features and treatment status of corticosteroid-dependent dermatitis.Then collect the data and statistic analysis.(2)Clinical observation:A multicenter, randomized, open study was carried on.64patients with corticos-teroid-dependent dermatitis, which was divided into experimental and control group. The patients in experimental group were topically treated with Intensive Moisturizing Cream containing prinsepia utilis royle oil, purslane extrac twice a day after washing face with plain water combining with topical tacrolimuso intment0.03%once a day, and the control group only was topically treated with tacrolimuso intment0.03%once a day for28days. Then, the patients in experimental group still topically applied with with Intensive Moisturizing Cream containing prinsepia utilis royle oil, purslane extrac twice a day after washing face with plain water for28days. The lesion score, TEWL value, the water content and the sebum content of stratum corneum was measured, and the improvement and reoccurrence rate were evaluated in7,14,28and56days after treatment.Results(1) Clinical analysis:192cases of patients with corticosteroid-dependent dermatitis.166female cases accounted for86.5%,56.8%patients were20--40years old in youth,165cases (86%) with higher culture degree.The average duration is20.13mouths.77cases caused by drugs.Piyan Ping to be used up,with19cases accounted24.7%.115cases caused bu cosmetics,and43cases of freckle products accounted for37.4%.A11the patients could feel the sense of tension and burning sensation,which109cases (56.%)were manifested as facial dermatitis type,41cases (21.4%)were acneform dermatitis type,20cases(10.4%)were pigmentation type,16cases (8.3%)were aging skin type,6cases (3.1%)were vellus hair hyperplasia,and56cases were mixed type.28cases of patients (14.6%) without any treatment.87cases of patients (45.3%) used oral and external medical treatment,and the effective rate was31%.42cases of patients (21.9%) used medical skin care products and its effective rate was38.3%.55cases of patients (28.6%) used the beauty treatment and its effective rate was83.6%.(2)Clinical observation:①the improvement of clinical symptom:compared with pretherapy, the lesion score of the experimental group decreased in7(1.73±0.69),14(1.23±0.68) and28(0.93±0.87) days after treatment (P<0.05).Compared with control group, the lesion score of the experimental group significantly decreased in28days after treatment (P<0.05);the improvement effectivity of experimental group was higher than that of control group in28days after treatment (P<0.05);②The outcomes of the physiological function of the skin:compared with pretherapy, the TEWL value of experimental group decreased in28days after treatment (21.38±6.57), the water content of stratum corneum of experimental group increased in14days after treatment (30.92±5.63,P<0.05), the content of the fat increased in the28days. Compared with the control group, in28days after treatment the TEWL significantly decreased and the water content of stratum corneum significantly increased (P<0.05). ③self-assessment:in the experimental group the feeling about the smoothing moistening aridity tightness burning sensation and itching improved obviously better in the14,28,56days (P<0.05),and also batter the control group.④the recurrence:Observed after the therapy stop for28days, we have found the recurrence of the experiment group is3.7%, and27.3%in the control group.⑤the evaluation of the safety:there was no adverse reaction in the two groups.ConclusionIntensive Moisturizing Cream containing prinsepia utilis royle oil, purslane extract, combining with tacrolimus ointment0.03%could improve the effect of treatment of the corticosteroid-dependent dermatitis and reduce recurrence of corticos teroid-dependent dermatitis.Piyanping ointment and freckle cosmetics are the inducement that causes corticostero id-dependent dermatitis which manifested as facial dermatitis-type as the most common clinical type.Medical skin care products and beauty treatment can improve the curative effect.
