Relationship Of Progesterone/estradiol Ratio On Day Of HCG Administration And Pregnancy Outcomes In In Vitro Fertilization

Objective:This article aims to explore the predictive value of a serum P/E2ratio measured on the day of HCG administration regarding pregnancy outcomes in high responders undergoing IVF/ICSI-ET, so that better dicision and prediction can be made.Methods:A retrospective analysis was conducted in400cycles in which GnRHa protocol was applied in our center from January2011to December2011. Among the400cycles,246had in vitro fertilization and embryo transfer while154had intra-cytoplasmic sperm injection and embryo transfer. Eligible infertile women undergoing IVF were assigned to four groups according to serum P levels on the day of HCG administration:group1, P≤1.0ng/mL; group2,1.0<P≤1.5ng/mL; group3,1.5<P≤2.0ng/mL; group4, P>2.0ng/mL. The relationship of E2level and P/E2ratio on the day of HCG administration and pregnancy outcomes was analyzed.Result(s):1. The E2levels and P/E2on the day of HCG administration increased as P level increased.2. The P/E2ratio on the day of HCG administration have been found to correlate negativly with the pregnancy outcome3.Patients in group4had highest P/E2ratio, as well as lowest implantation and pregnancy rates.Conclusion (s):1.Using the level of a single sex hormone on HCG day to predict pregnancy outcome in high responders undergoing IVF is confounding, whereas using a P/E2ratio on HCG day is more reasonable.2.The P/E2ratio on the day of HCG administration can predict the outcome of patients with high ovarian response, when the P/E2ratio is too high, the implantation rates and pregnancy rates decreased significantly.
