Report On The Practice Of Clinical Pharmaceutical Service In The Affiliated Hospital Of Yanbian University

Objective:With the development of medical and health care in our country, ensuring rational drug use in patients and improving the drug therapy have become the key tasks for the medical staffs. By participating in the clinical pharmaceutical services in Affiliated Hospital of Yanbian University, the author summarized the current situation, achievements and deficiencies, and gave advices. The present study aimed to provide evidence and reference for enhancing the level of clinical pharmaceutical services, thus, adapt to the rapid development of clinical pharmacy under the new situation.Method:Based on the pharmaceutical intervention of rational drug use in digestive diseases and clinical application of antibiotics, the effects and improvement approaches were summarized by participating in clinical ward round, pharmaceutical care, prescription comment, medical consultation, pharmaceutical education, ADR monitoring, and the clinical application of antibacterial drugs special rectification activities in this hospital. The prescription proportion of antibiotics in outpatients, the antibiotics utilization ratio and antibacterial use density (AUD), the ratio and amount of perioperative preventive use of antibiotics in type I incision operation in hospitalized patients, the top10antibiotics on the list of sales amount were analyzed comparatively, thereby, the situation of clinical pharmacy work in this hospital were fully analyzed and investigated.Result:Level of rational drug use was enhanced, and the various indexes for application of antibiotics were significantly improved by promoting the clinical pharmaceutical services in this hospital. The ratio of outpatient antimicrobial prescription obviously decreased from24.42%to11.75%(P<0.05). The ration of antimicrobial drug use and AUD in hospitalized patients reduced from64.18%,50.69(defined daily dose, DDD) to51.33%42.78DDD, respectively. The ratio of perioperative preventive use of antibiotics in type I incision operation significantly decreased from88.2%to88.2%(P<0.05). The structure of drug usage obviously changed before and after improvement.Conclusion:By performing clinical pharmaceutical interventions in this hospital, the level of clinical rational drug use and medical quality were improved, and a more secure, effective, economic and reasonable drug treatment would be provided to the patients. After the clinical application of antibacterial drugs special rectification activity for3years, the management of antibiotics use was strengthened; the responsibility system for clinical application of antibiotics, the classification management system, and the clinical case review system were established; the training and various indexes for rational antibiotics application were significantly improved. In results, the clinical application of antibacterial drugs special rectification activity in this hospital was effective, which improved the rational clinical application of antibiotics. Therefore, there are still some problems, such as clinical pharmacists’lack of medical knowledge, no national standardized regulations for the clinical pharmaceutical service, low acceptance of clinical pharmacists by doctors. Meanwhile, by systematic study and participating in the clinical pharmaceutical work in this hospital, the author improved the medical knowledge, also comprehended that the heavy responsibility and task which clinical pharmacist should take. As a clinical pharmacist, everyone should improve the professional level, and play a positive and critical role in medical team, thus to relieve disease and give healthy for patients.
