Purpose To perform a meta-analysis of all prospective RCTs for the recent ten years to quantify the potential benefit of chemotherapyafter complete resection over surgery alone in terms of overall survival,disease-free survival,relapse-free survival.Methods Search the electronic databases including CNKI,PubMed,Elsevier,Springer,Oxford from Jan2003to Dec2013,then give a quantitative analysis of the HR and95%CI of OS,DFS,RFS with RevMan5.2.Results There are ten prospective RCTs with4points and moreof Jadad for this study,including4433patients,2203in the chemotherapy group and2230in the surgery alone group.After statistical analysis we conclude that the life of patients can apparently benefit frompostoperative adjuvant chemotherapy.OS:HR=0.80,95%CI(0.73,0.88),P<0.00001;DFS:HR=0.78,95%CI(0.63,0.97),P=0.02;RFS:HR=0.67,95%CI(0.53,0.85),P=0.001. Conclusions Postoperative adjuvant chemotherapy can improve OS,RFS and DFS,perhaps it is related to the TNM stage or the correlation factors.Capecitabine plus oxaliplatin and S-1alone maybe the bestadjuvant chemotherapy plans after D2radical correction of advanced gastric carcinoma nowadays.
