Objective:Combining with traditional method and the angle of tibial torsion to make sure the rotational alignment of tibial prosthesis in total knee arthroplasty. In order to achieve the tibial prosthesis placed the correct rotation and better post operation effect.Method; Treat64patients (72knees) with total knee arthroplasty during2011.5to2012.12. Male:21cases (22knees), female:43cases (50knees). Age:55-78years, average age62.43years, the patients were split at random into two groups. A group:28cases (30knees), B group:36cases (42knees).At3days before operation,3days,1month,6months after operation, evaluate patients with KSS, HSS and pain visual analogue scale (VAS). Group A use tibial tubercle in1/3line as the anatomical landmarks; group B use tibial tubercle in1/3combined with the angle of tibial torsion as the anatomical landmarks. All were measured for the same physician.Results:All patients were effective follow-up, one month after surgery:HSS score:A group was86.1±3.5, B group was84.5±4.3; KSS score:A group was68.2±5.8, B group was66.4±6.7; VAS score:A group was3.2±0.7, B group was3.2±0.4.6months after operation:HSS score: A group was80±6.8, B group was80.1±4.2; KSS score:A group was63.4±4.3, B group was63.5±3.7; VAS score:A group was3.4±0.5; B group was3.3±0.3. Patients in group A and group B compared3days before to after1months and6months was statistical difference (P<0.05), compared6months after operation to1months after operation was no statistical difference (P>0.05). Comparison between A group and B group HSS, KSS, VAS score:after1month and6months after operation showed no significant difference (P>0.05). In patients with postoperative1month over the3days before operation to improve the situation, in A group, B group showed no significant difference (P>0.05), compared improving of6months after operation to1months after had significant difference (P<0.05), no significant statistical difference in the VAS score (P>0.05).Conclusion:we find that use the angle of tibial torsion as a new kind of tibia prosthesis rotational alignment positioning is feasible. But its long-term effect still needs further discussion.
