Objective:to investigate the characteristic of three-dimensional geometry of mitral annulus and leaflets complex in ischemic mitral regurgitation and predictors related to the degree of regurgitation. Methods: two-dimensional echocardiography images and real-time three-dimensional transesophageal echocardiography images were collected by Philips IE33ultrasonic diagnostic instrument. forty patients with ischemic mitral regurgitation were divided into two groups:group of mild mitral regurgitation and group of severe mitral regurgitation according to the degree of the regurgitation. twenty healthy subjects without mitral regurgitation was considered as the control and post-treatment with MVQ analysis software acquired three dimensional parameters of mitral annulus and leaflets complex. Results:compared with control groups, C3D、 DAIPM、PAL、NPA、L3DEA2、L3DEP2、L2DAIPM、LC3DLF were increased in both significant mitral valve regurgitation group and mild mitral valve regurgitation group, and the parameters is increased in significant mitral valve regurgitation group are more than mild mitral valve regurgitation group; compared with mild mitral valve regurgitation group and control groups, A3Dmin、AAL、DAP、θnpa、A3DEant、 A3DEpost、Htent、Vtent were increased and H were decreased in significantly mitral valve regurgitation group, but no significant difference in between mild mitral valve regurgitation group and control groups. Conclusion: the change of mitral complex geometric configurations is closely related to the generation of mitral regurgitation, and along with degree of regurgitation aggravating, deformation more seriously; Simply left ventricular remodeling without significant reshaping of mitral valve annulus and leaflets complex does not cause significant ischemic mitral regurgitation.
