Objective:In this study, an erbium, chromium:YSGG (Er,Cr:YSGG) laser was applied to remove root caries in the elderly, the laboratory study was observed of the microleakage between the restoration and cave walls, the clinical study was pain assessment for cavity preparation, to provide theoretical for Er,Cr:YSGG laser in clinical application. Methods:(1) Fourty in vitro teeth which has the root caries were treated as following. Observation group:removal and cavity preparation with Er,Cr:YSGG laser. Control group:caries removal and cavity preparation with dental highspeed air turbine. Each group has twenty teeth. Then the observation group randomly divided into two groups, group A using Fuji II glass ionomer filling, group B using self-etching composite resin filling, the control group filling method as same as the observation group. Using thermodynamic cycle method and dye penetration the completed filling teeth in vitro, combine with a stereomicroscope and scanning electron microscope (SEM) observed the microleakage.(2) Select the patients with root caries who’s age of50-60years old,15male and15female, totally of60teeth. Each patient had at least two teeth of which caries degree were similar (the left and right side or maxillary and mandible). Caries was removed and cavities were prepared using Er,Cr:YSGG laser technique (experimental group) and traditional handpiece method (control group) respectively. The pain degree of each patient was recorded with Face Pain Scale-Revised after the treatment. Results:(1) The in vitro teeth were filling with Fuji Ⅱ glass ionomer and self-etching composite resin, there were no significant differences of the dye leakage among two groups (P>0.05), there were no significant differences of the gap width between the micro crack filling body edge in the carnal wall and gingival wall (P>0.05).(2) Er,Cr:YSGG laser can alleviate pain obviously during the root caries removal and cavities preparation compared with traditional method, the difference was significant(P<0.05). Conclusion:(1) Er,Cr:YSGG laser does not reduce the microleakage between filling body and cave walls afte root caries remoxed and cavity filling.(2) Er,Cr:YSGG laser can alleviate treatment pain and discomfort obviously during the root caries treatment, so it is a new technology and choice for microport painless treatment of the root caries.
